Brilliant video guys but can you just clarify the following please. At the end of the video you state if 1 from 3 is positive and if 0 from 3 positive this indicate ...... however arent we looking at more than 3 items here? Am i missing something?
The study said that there is a sensitivity of 96-99% and a specificity of 26-47%, and then you make the conclusion, that "if the ottowa ancle rules are negativ, chances of a fracture is low", and that is not korrekt. For that to be true, we need a high negativ prediktive value (NPV).
Hi Johanne, the problem with the NPV is that it depends on the prevalence and to exclude a fracture you'd want to end up with a very low value. Best thing to do is to take the negative likelihood ratio, which is 0,15 (worst case) and 0,02 (best case) if we calculate it from the sensitivity and specificity. So if we assume a prevalence for a fracture of 15% and calculate with the likelihood, we'll end up with a chance of a fracture somewhere between 0,35% - 2,5%.
For the Ottawa ankle rules, you forgot to mention to palpate the navicular bone too for pain which is also indicatif of an ankle fx, otherwise awesome video!
