An addendum to my previous post. There's several medical issues that could cause a major change in personality and mental behavior. Like an Illness or a tumor in the brain which can put pressure and damage the brain structure. Another possibility is trauma to the head where the person literally becomes an entirely different person.
The sister's ex boyfriend sounded like he is addicted to drugs or alcohol or both. Another possibility is trauma related. Where a situation or person brought back memories of an abusive relationship. In some instances the person who either was abused or witnessed the abuse against a family member. In that case there's several different responses that could happen. One is repeating the abusive patterns as either the victim or as the abuser. Often the child will react as a protector to the abuse victim as a young child but as they get older there's a switch from identifying with the victim to identifying with the abuser. Obviously this is only one possibility along with many other possibilities that can and will happen but that's why getting help through counseling and talk therapy is important. With treatment the positive outcome can be carefully curated in order to stop the generational gift that no one should ever give or receive to anyone.
