Is the underfoot cushion all from the insole? What is inside layer after you remove the insole?

There is another foam pad just above the sole of the boot. You can remove the insole and add something else if you wish.

These are so hard to put on they are nearly unwearable.

I find Chelsea boots are quite tight at first but after a few months the elastic stretches out. You might want to try shoe trees too.
