    I have read past 3 editions and i think there is a desperate need of an editor, for both of your books, step2 and step3. The knowledge is haphazardly placed in chapters.

    Sir, the book is missing the topic Polycystic kidney Disease, despite it being mentioned in the index (says its on page 392, but nowhere to be found :( )

    Dr Fischer, on page 431 of your 6th edition, it states: "pregnancy ... nor does it change normal values/ranges of free serum thyroxine (T4) and TSH. Please elaborate?

      I believe this is due to the fact that during pregnancy that there is an increase in thyroxine hormone binding globulin. The globulin sequesters away free hormone, more thyroxine bound to protein, less free thyroxine. Then TSH is not as inhibited as before, so more thyroxine is stimulated by TSH until free hormone reaches a normal range. Once the free hormone feeds back to the pituitary, TSH is inhibited. Overall you have increased total thyroxine, both bound and unbound combined, but the free thyroxine stays the same. The pituitary gland only respond to the free thyroxine.
