When the game was released in Japan, it had to be altered because students cutting classes was a huge problem in Japan. Instead in the Japanese version, you play as a blue haired office worker named Tooru and everything has been edited to look like an office setting in the Japanese version with the exception of the basketball court. Konami before this had used licensed songs from other sources, however they didn't have the rights to use said songs which put them in risk of getting sued by record companies. For Mikie however, they got permission from the JASRAC to use "It's A Hard Day's Night" legally.
I played this on MAME yesterday and my god is it hard. You got to time the head butt perfectly, and the AI moves and can catch you pretty fast. The remixes of A Hard Day's Night and Twist and Shout are pretty good, though.
There's one game you'll never see on Virtual Console. Two songs were Beatles songs. ("A Hard Day's Night" and "Twist and Shout"; the latter wasn't their song originally, but they made it more popular than ever)
Makes me wonder if they will end up getting the game muted or copyright claimed by UMG UK, who has been giving false copyright flags on anything with Beatles music. Another reason this will never be seen on Virtual Console or anywhere else is because some stupid parents will complain it will teach kids to skip class, defy school authority, and even for influencing kids to start bullying other kids.
I hope so, Vahan. Those false flags are the reason Paul is taking them to court in order to get the catalogue back. But then again, in this age of TH-cam, companies have been known to get overprotective of their stuff.

The attract music stuck in my memories for years and I was delighted years later when I could see the game again on MAME.

At the time of release, there was a lot of school violence in Japan, so they didn't want to promote that kind of thing.
Pretty much, some stuff is a little weird by US standards like the "restaurant" (should be "cafeteria", unless the school had a class to run such a place like the one I went to) and perhaps "Garden" ("Campus" would be more like it). Originally in the Japanese version, you're a Salaryman escaping the job at a office building. I guess changing it from that to an American high school was the best Konami could do.

Honestly, I never understood the theme of this game - kicking innocent students out of their chairs, unauthorised visits to other areas of the school, headbutting members of staff - he'd be permanently excluded in no time these days...

This version was Americanized from the Japanese original, two years before Kunio-Kun/Renegade did that.
