    Eastern Airlines Flight 401 crashed into the Florida Everglades while on approach to Miami International Airport. The National Transportation Safety Board (NTSB) determined that the crash was the result of an inadvertent autopilot disconnection that went unnoticed by the flightcrew as they were attempting to correct an unsafe landing gear position indication. The NTSB determined that the uncommanded descent into the Everglades was the result of the flightcrew's failure to monitor the airplane's flight path and an improper division of duties on the flight deck while troubleshooting an anomalous system indication. Of the 163 persons on board 112 were killed in the crash. This accident was one of the precipitating accidents leading to the development and industry-wide adoption of flightcrew resource management philosophies and training.
    (Note: This reenactment was filmed in a simulator, and was not produced by Eastern Airlines. The simulator used for the filming was not identically configured to the flight deck and warning system originally delivered to Eastern Airlines. In this video, following the unintended departure from 2000 feet, a warning tone is heard, and footage of the altimeter shows a continuously flashing amber light. Eastern Airlines had inhibited the flashing alert light below 2500 feet above the ground, so on the accident airplane, only the tone was produced, at 1750 feet. There would have been no flashing light. This discrepancy in the video could lead the viewer to assume that the flight crew must have ignored, or missed, two warnings, while in reality, the only warning that would have existed would have been a single, short duration, aural tone.)

      Because how the plane came down level. They ended up using the parts in other planes of the same type and every last plane they put the parts in had apparitions of the 401 crew appear and disappear in front of numerous passengers and flight staff. Even in one instance the 401 Flight Engineer appeared to the captain of another plane with the 401 parts and warned him that his plane had a malfunction in one of the engines before disappearing in front of the pilot. The pilot was startled but had the mechanics check the engine and sure enough it had the exact malfunction that was described to the pilot by the deceased 401 flight engineer. The 401 captain appeared on another plane that his very close friend flew that happened to get parts from the wrecked 401 plane. The friend of the 401 captain seen his face, immediately recognized him, fell backward into the isle and the 401 captain disappeared in front of all the passengers sending them jumping out of their seats in a panic, crying hysterically, etc. Another instance the 401 captain appeared in an empty seat next to a woman passenger and then disappeared as she watched and others watched, she screamed, and she was so disturbed by it they had to remove her from the plane in a straight-jacket. Not joking.

If you assume the ghost stories are true, they make sense since the crewmen probably would have had an extreme sense of guilt for having screwed up so badly and so stupidly. Prayers for everyone involved in the tragedy.

    ... and the most important point is that when you have a 3 man crue + a technical officer returning home, all of them should not be focused on the same problem and someone should fly the plane.

      Most of these accidents you see are all caused by human error, at least 95 percent of them or so... Sad!

      The reading of the controller is imperfect. It is delayed and can give out wrong readings and it takes some times to update to the correct reading.
      It is not the task of the ATC controller to provide ground avoidance.
      ATC also has other task. So, ATC saw an anomaly that could be a system glitch, asks if everything is OK and after getting an OK, ATC moves on to regular duty.

    No one noticed the warning or that the altitude dropped so fast. Scary! I thought airlines require one pilot on duty in the cockpit at all times. Seeing that this is a movie, it is eerie.

      This is before the concept of crew resource management existed as a mandatory policy at airlines. This is one of the major crashes that brought that idea to light.

      Yea, apparently the planes that used the spare parts from this flight got multiple reports of ghost sightings on the planes, but when eastern removed the spare parts from the planes, they ghost sightings became less frequent and eventually stopped

    On the TriStar dash [this demo cockpit is a DC10] you can also interchange a main gear light with the nose gear light and vice versa, to check if it's a faulty bulb or light assembly. I've had this situation once, that's why I know about it.

    A lot of things could have been done but ATC didn't drop the ball here. In fact the controller went above and beyond his duty by asking Eastern 401 how they were doing after their altitude dropped to 900. ATC wasn't required to contact pilots at all under those circumstances before this accident occurred because at such low altitudes the equipment of that era often reported incorrect flight levels, sometimes even CST ("coastal" aka 0 FT), when in reality the plane hadn't descended at all.

      @@kathleencastle8631 Yes, that is technically true. But as I said, the equipment often wasn't reliable. The Tristar was brand-new, so it transmitted accurate C-mode information. But the controller had no way of knowing that for sure, and it probably wasn't the first time his radar scope showed an unusual altitude, especially as a plane traveled further away from the airport at a relatively low (2000 feet) altitude.

      If the ATC was going to make the effort to contact them because he was concerned about their altitude change, it only makes sense that he should have alerted them to it to get a confirmation of what it was. If he was concerned about their altitude he did an aweful job of addressing the issue with them. Why did he even bother. What was legally required per the rules and what a reasonable ATC would have done are 2 different things.

    I read about this once. Like a lot of infamous disasters it was a perfect storm of various weird things coming together to let it happen the way it did.

      @@stickapot3623 .. I find it strange that ALL THREE pilots focused on a faulty indicator light and no one was monitoring instruments. If just one pilot had noticed the autopilot wasn't engaged or noticed the altimeter changed they could have easily made the necessary adjustments. CRM became a priority after tunnel vision caused fatalities. Lessons were learned but the price was steep - so many perished.

    Thankfully, nowadays, we have alerts that sound when the AP is disconnected. If these poor lads had the same technology as today, they most likely would have survived.

    Ive watched NatGeos (?) Air Crash Investigation a lot and this was one of the crashes shown on the series. It never seizes to amaze me how everyone is focused on a this problem and noone is flying the plane. There were a few other incidents like this on the series. Then again mistakes obviously happen and sometimes grave ones. Still, every pilot should know to first fly the plane and then troubleshoot.

    Cockpit Chat: "Hey does any of you guys want to climb outside and take off the exterior protective cover from the landing gear porthole?"
    "Hey, what was that chime?" -- "Dunno, probably nothing."
    "Hey, we're still at 2000, right?" -- while looking at the altimeter showing 700-ft. or less.
    Miami ATC Chat: "Hey, how are you guys doing out there. Still at 2000 -- or have you guys just decided to land in Everglades at the stroke of midnight?"
    Anyhow, seems like half the airlines crashes in this period were logged by Eastern Airlines aircraft and pilots.

    Nothing fixes stupid, if it can be said. 3 crew members in a wide body jet was standard at the time (Flight Engineer) and there was a "Jumper" (Another flight crew Eastern Employee) in the 401 cockpit who was heading back to Miami. No one was watching the panel gauges!! The other problem is different aircraft manufacturers have different flight control equipment inputs. I had read the L-1011's Autopilot was disconnected by slight movement of the yoke. This was presumed to be the reason the AP became disengaged. The reason for it: If either pilot grabbed the yoke for a sudden movement, the AP would be disconnected. Here, it was a recipe for a fatal end in this case. Nothing was wrong with the L-1011 design, but it's reputation was damaged because of this crash. The new L-1011 was the most sophisticated Wide Body Jet of the 3 then. It had "Auto Land" capability--way ahead of its time. 3 Wide Body Jets: Boeing 747; MD DC-10, and Lockheed L-1011.

      It had "TCS" Touch Control Steering......the F/O bumped the yoke forward, enough to put it in a 100' per minute descent. The TCS holds the descent or climb rate that you set by moving the yoke forward. At the time there were no altitude warning lights to tell a pilot the he was either approaching his assigned altitude or was deviating away from the set altitude.

      I’m guessing you meant incompetent. The F/E did go down along with the jumpseater. Crew Resource Management has resolved a lot of the issues of who does what in a situation like this. It’s actually saved lives.

      somehow there was a minor electric failure and the auto pilot got off when the electric failure was fixed the auto pilot was off instead of on as it was and they didn't checked the altimeter untill it was too late.

    Yes I know it is NOT his job but really---the controller knows that they suppose to be at 2,000 and he sees them at say 600 AGL. What the f....say something like; "Easter 401 I show you at 600 AGL, what is your status!" after all the Miami area can be congested there may be conflicting traffic in the are. Just saying of course hindsight is 20/20 and yes I did control at one time..

    The “forward forward” at the very end was what was said just before the Air Florida 737 went down and smashed into the bridge in Washington, D.C. (not sure why it was added to this one???)!
    On another note, all the ATC had to do was mention that their altitude shows under 1,000…& there wouldn’t have been a crash!!!

      @@galady8632 Is it true that the autopilot switched modes (not disengaged) and it was determined that the captain might have nudged the switch on the yoke or stick (not sure what kind of configuration there was on this aircraft) when he turned in his seat to speak to the flight engineer who descended to check on the landing gear? I thought I read that somewhere. If so that is kind of crazy in itself, a rather ludicrous design flaw if you ask me!.and especially at just the wrong time too. Not saying the crew were not inept or at least brain farting with not one of 4 guys bothering to monitor the instruments!!!

    @TurbulentG In fact, they performed a christmas tree test or whatever its called, that lights every warning bulb in the cockpit, at that point, the captain could have assumed a faulty bulb, fact is, he was grumpy and irratable, and when asked by his copilot should he take over, he ignored him and kept focusing on the light, while accidently touching off the flight controlls, so its not a case of shit pilots, its more a case of 1 captain who gets annoyed and angry, and a crew that follows orders

    Everybody dropped the ball here, the pilot is supposed to fly the plane, NOT THE AUTOPILOT, maintenance problems are the flight engineers area of responsibility, NOT THE COPILOTS, and finally, it would have been really nice if the ATC had asked them to pull up just a little and told them they were below 500 feet so the delay in comprehension wouldn't be fatal.

      ATC did not know the flight height. ATC only saw a radar display value, which could be incorrect and is dealay. all ATC could have done is ask for their flight level, to which the reflective reply would be "flight level 2000 on auto pilot", which would have confirmed to ATC that the radar reading is incorrect and that he has to focus on other planes.

    They found in the wreckage that the front gear was down and the plane could have landed with the front gear up anyway.

      They could have easily landed without nose gear and barely noticed a difference except for a loud scraping noise.

      thomsonfly645k this is back in the 1970s, so technology still weren't advanced and there is not much warning sound in the cockpit, the altitudes warning did sound but the crew didn't hear it

    i heard two aircraft crashes that happens with this plane. the first one is eastern 401 which is caused by pilot error and CFIT (controlled flight into terrain) and delta 191 which is caused by a microburst and pilot error
    please note me if there was another crashed that involved this type of aircraft

      Microbursts are rare and at the time, most commercial airline pilots never even heard the term, let alone knew how to detect one and safely maneuver around it

    No doubt the pilots did not monitor the important instruments, especially important without visual references at night. I guess a very gradual descent could be missed when other distractions are going on, but you would think experienced pilots would have a much higher awareness that they are in an aircraft that has to maintain an altitude. Seems kind of unbelievable to me especially with two pilots, apparently neither having that proper awareness and in an aircraft they were familiar with.
    Inadequacies seem like no altitude warnings in addition to the first one? and certainly beyond simple chimes in headphones. Also as many important indicators as possible should have multiples bulbs so we don’t have crews so preoccupied with a malfunctioning indicator. Another problem that I feel has always been a problem and is even worse every day is the reliance on electronic flying controls. Too much reliance on systems that usually work, engendering excessive trust, but that are not always reliable and certainly not perfect.
    Tragedy and hopefully some things were learned.

    they not only rely to much on auto pilot but I disagree I think ATC was wrong in not being clear in what they ment. asking how they were doing is way to vague when you fly a plane. He should of told them out right to gain altitude, stating there current altitude. Over 100 lives could of been saved had he acted professional on the job instead of asking a vague question like that.

    The comments below that state "shit" pilots is not a fair call. Think of this: It is 1971; no Pilots back then were ever trained in CRM; CREW MANAGEMENT; the duties were not delegated properly. While the Captain was agitated, his lack of training showed. These days we are aware in any industry of how to deal with adversity; like a grumpy Captain that neglected to answer the Co-Pilot when he asked" should I take over on flying plane...it was like the old Pollack joke:how many does it take to fix

    auto pilot shut off by accident (to much going on in the cockpit), loss of altitude, noboby notices (too much going on in the cockpit). cartwheel into the glades in the middle of the night. lots of heros respond. i remember the a.m. radio news flash well (wqam). RIP. don repo lives on in an oven on 318. keep on keepin' on.

      2000 was just fine. Pilots have two things above all else to watch. Air speed and altitude.
      Way too many crashes that could have been avoided because pilots weren't doing their job and became distracted. this has even happened at 35000 ft so no, altitude wasn't the problem. stupidity was.

    I try not to judge because I don't have the stick time but three crew member didn't look at the altitude for many minutes at 2,000 feet near a congested area at night is just not acceptable. Too much talking in the cockpit and no one is flying the darn plane. Captain I'm sorry to say this is on you! May God rest those who died that day!
