If I wanted the smoothest nib available in an EF (or at most a F) what should I be looking at? But if I wanted to see what really smooth felt like in my preferred line width, what should be on my wishlist? (I already have such pens as Lamy Safari EF, TWSBI 580AL EF, and Pilot Metropolitan F)
Japanese nibs are generally finer, yes
Pilot makes great fine nibs, I love the Custom 74, Custom 823, Vanishing Point, E95s, Stargazer, and Justus 95
Pilot makes EF nibs in a few pen models, they’re REALLY fine but you sacrifice some smoothness to get that
Platinum Preppy EF is remarkably smooth for how fine it is
Faber Castell EF is actually pretty great!

The Platinum Preppy is an excellent standard for Fine and ExtraFine. I've got a set of these in F and a couple EFs, They're all mixed up now and I can't tell the difference. They write exceptionally well, I use them for corrections and tiny margin notes because they write so finely, and I've converted a couple to eyedropper. I've never had a bad Pilot or Platinum nib. Some of the Pilot EF's are a bit scratchy-but they're almost like hypodermic needles. Give a kid a Platinum Preppy or a Pilot Varsity when they're in first grade and they'll have happy blue fingers for the rest of their life.
I love my EF Platinum Preppy. Super fine and pretty smooth - well worth the $5. EF Lamy Safari is a bit scratchy but not terrible, but also not as fine. Can't wait for a fine nib Pilot Vanishing Point, but it costs a little more.
The EF steel nib in my Edison is wonderful, and the soft fine 14k nib in my 3776 is great with a surprising amount of spring.

I would recommend checking out Franklin-Christoph 0.25mm needlepoint nibs. I mean it's a very hard nib, but if you're looking for a high quality nib that will give you an exquisite hair fine line. You won't be disappointed.
Facts! I tried a demo at the recent Dallas Pen Show, and it was amazing.
The Visconti Rembrandt in extra fine is a beautifully smooth writer.
Smoothest is likely going to be something like a Pelikan 14k/18k or Visconti 21k pd. nib. But, the reason they will feel smoother is because they write more like a medium than an extra fine!! The smoothest "true extra fine" I own is a Lamy 2000. Keep in mind that the pen does have a small sweet spot, so if you aren't writing with it aligned properly you won't experience the smoothness. But, when aligned correctly, I feel almost no resistance to the paper. It's not going to be as fine as a Japanese EF/F but I would still call it a pretty solid EF. I have a Pilot Custom 74 in a medium and it is a fantastic nib so I'm sure going with a similar pen in a EF or F would also make a great writer, as Brian suggested.
My experience with Pilot's EF, and F nibs is exactly the same as Brian's. The F is incredibly smooth. Especially on my Pilot E95S. I have another Pilot E95S but in EF, along with an EF Custom 74. With the EF nibs, you definitely get some feedback. Not a whole bunch but it's there and the F is definitely quite a bit smoother. As much as I love my EF Custom 74, yeah; I do wish I had gotten it with the F nib instead of the EF. I also have a Pilot Metropolitan in F. Despite being a steel nib while the others mentioned above are 14k gold nibs, the metro is also surprisingly smooth. If smoothness is what matters most, go for the Pilot pens in F.
Jowo 18K EF and Omas 18K EF are my favourite for now.

How can you get ink out of the grip section of the custom 74
+Patrick Unger it's technically two pieces there, but they're glued in place. I don't know why it is that way! Mine has come apart over the years so I can unscrew it on demand, new ones don't do that though. -Brian Goulet
what is that “feedback” that the Preppy has?
The EF Preppy is one of the finer pens I've written with, so it can dig into the paper a bit. Not enough to feel scratchy, just grounding you when you're writing letters. Sometimes with super smooth nibs you can lack the control on each letter. - Colin

Faber-Castell Ambition extra fine or pilot metropolitan fine... which would be a better option for smoother writing??? (Ignoring the price difference)
Both have some noticeable feedback in my experience. The JoWo EF nibs are smooth, but will write thicker than the Japanese EF. -Lydia
Sailor and St Dupont also make pretty smooth nibs.
I am stuck between buying an EF nib for my Lamy safari or a Faber Castell loom. What do you think? (The finer the better)

Faber-Castell (I don't know about Graf von Faber-Castell) nibs are just plain old Jowos with a purely cosmetic makeover. It's a very standard European nib sizing system, so there's not really that much fineness.
Im very confused about when you guys say "smooth" because I own the Pilot Metroplitan in Medium and I wouldnt classify it as smooth....when you guys say smooth do refer to how easily ink flows onto the paper? Or are you refering to the tactile feel of the nib on the paper?
The reason I ask this is because my unibal signo is way more smooth than the pilot even my g2 pens are smoother than the pilot metropolitan
We are referring to the tactile feel of the nib on paper. You're not exactly comparing apples to apples with the gel pens vs fountain pens. The Metropolitan does offer a smooth writing experience for a fountain pen. More so than other brands like Platinum/Aurora/LAMY, especially with finer nibs. - Colin

Those are more in order of price. I'd say the LAMY 2000 in Broad is the smoothest fountain pen I've ever used. - Colin

I used the same ink in all of them and wrote on the same paper.
Several times. So I hope I could rule out myself as a cause for this.
My extra fine is smoother than Pilot metropolitan M nib at its sweet spot ( it does have a narrow sweet spot)... While it writes in between pilot f and m width.. after 2017, EF you get on pens in Lamy ( not spares) are thinner and smooth with sweet spot.. you definitely had great luck👍
