Good guide, I would also put emphasis on DOTs - stacking them lets you win pretty safely when it goes into deep-breath territory its already over - DOTs finishing the body while you deflate lungs (so DOT trophies, fragment grenades etc help). But if you only have direct damage you might want to kill one lung first before pushing the main body to low health to avoid constant deep breaths. And you can use a tank to take all the blinding attacks making sure the team is safe and can always attack reliably - just like MA in the video. So yeah you better plan and prepare for it right at the hero selection.
I just got here for the first time, with a recruited Alhazred who was honestly jacked as hell, a mag-rain centered Paracelsus who carried healing with BM and indiscriminate science, as well as a rank 1-2 centered team of Dismas and Reynauld, absolutely kicking ass. Each one hit 10 stress, and got a resolute, back to back, only to get solely meltdowns after hitting deaths door a few times. Everyone was on death’s door, Paracelsus went down with a killer 6 point blight, and Alhazred went down at the last hit with a burning stars which was the sole reason I was able to beat the boss. As the narrator said his death line, Dismas nailed it with a pistol shot, and it was the most hard thing I’ve ever seen
Just got it done first try and I was just trying to get the crusader lol Got really bad traits but the rng gods blessed me with my boldness (a lot of the routes for the crusader were pretty dangerous but ended up with some insane trinkets) Used leper, highwayman, vestal and plague doc. Did it with your video info man, thxs. (The lung shit caught me off guard and the 9 and 12 health tip made the fight a breeze)
I just had my first try at this boss with Graverobber, Runaway, Crusader, Flagellant and it didnt work. I felt so op during the stages because my team had so much sustain. But since I couldnt put dots on the lungs it was over for me. Not enough damage. Sometimes I had to use all 4 heroes to kill one lung. Sadness
I’ve been away from the game for about a year. Right from when this boss launched in EA all the way to launch I never beat this thing because I just couldn’t understand the mechanics. Thank you SO much for giving an in depth overview of its mechanics. I am curious, and don’t believe you mentioned what happens when you kill one or both of the lungs?
This is probably my least favorite boss in the game because of how much it restricts your team building, and how it forces you to split your damage. Even after beating all 5 acts I'm still incredibly nervous everytime I set foot into its lair.

I talk about it a few times, it essentially makes some of the other attacks hit more heroes.
You also forgot the best trophies, which are the general's dream, to be locked at ranks or the safety of slumber, to fire resist
Dream only lasts for 5 turns now so its not good, burn on crit is probably the best trophy to just end it quickly

I think for this fight she was Venomdrop? But I don't remember
You forgot the Occultist. He cause good dmg at 1 and 4 rank. Also burning star ignores block token
