In the Last of Us, the original hospital seems to be worn down and gives off the idea that the surgeons don’t really know exactly what they’re doing, making the plausibility of a feasible cure questionable. In Part II, they remove that dirty feeling from the hospital and make it out as if designing the cure would be absolute. Joel even says in the prologue “They were actually going to make a cure.”
