His conjecture that some companies failed and some success while following the same processes. But he ignores the search for the Root Cause of both success or failure

Do you think that for example the Ishikawa diagram or simple CAPA can be easy applied in the context of a Complex or Chaotic System? Do you assume a possibility (and costs) of the full understanding of all the feedback loops, emergence, butterfly effects, strange attractors, etc. to perform true RCA? 3. Who cares about RCA if a bottleneck is removed? (ToC)
