@@ericjplays6136 I honestly feel the Duke is mystical. That is NOT a human size and his aura is non human when you first meet him He's high up looking down, both regal and neigh peasant level. He is massive and takes no damage. The Duke is likely some type of immortal being that sells and deals arms to all these creatures and has been doing so for centuries. They don't challenge him both out of respect and because he can't be damaged anyway.
That's my theory
Lady Dimitrescu lets Duke into her castle because they have business together. And because Duke is the only man large enough to handle Lady D.

More then likely infected, however due to plot reasons, the infection does not take him outright, instead, he seems to be fat and healthy, and huge. And impervious to explosions.... He is a chill infected.
We usually don’t get to know too much about merchants in games. It is part of what makes then awesome.

I think duke is just the same as what happened to lady dimetrescu, remember she got extremely tall because of the regenerative powers of the virus (cadou) and maybe duke was also infected with the same virus, pertaining to his massive weight and maybe, also regenerative powers as he literally does not care when ethan threw a pipe bomb?
