  • Vertical root fractures (VRF) and cracked tooth syndrome represent some of the most difficult clinical problems to diagnose and manage. CBCT, in conjunction with clinical findings, is extremely helpful in the detection, and treatment planning of fractures.
Additionally, with more options of endodontic and restorative materials, proper management of cracked teeth can allow patients to maintain their natural dentition more predictably than ever before.

Is it possible that a fracture is undetected even with a CBCT scan?
Tried root canal procedure twice with no luck. My CBCT scan showed no fracture in tooth but my pain is not going away. I went to the best doctor in town and even she is unable to manage my pain.
My canals are cleaned so the only possibility is a crack which I ruled out by a CBCT but I need to know that are there any fractures that doesn't show in the CBCT scan?

Any luck? I have a similar problem. Tooth pain from trauma, I can't even figure out what exact tooth hurts. Had endodontist examine, x-ray and 3d scan showed no problems.

@@onewithnature8435 yeah same here. Unfortunately such tooth had to be extracted eventually. I suffered lots of pain and spent lots of money or RCT and re RCT but in the end had to extract it.
The maxillofacial surgeon told me there are some cracks that are so complex that they are impossible to be seen in any scan. These crack opens when we bite from that tooth. Then closed back so an x ray can not see them.
I hope you get some solution or else you ll have to extract it I afraid but really after extraction you ll be relieved. It will be some intense pain for few days but you ll be given antibiotics and painkillers. Once it heels you ll be glad that you have it removed. My best wishes. For any questions feel free to ask.

@@Kashif314 I just saw the third dentist today and he didn't find any problems either. He believes it just needs time to heal. I have been chewing my food very lighlty and have had the pain go down to a magageable level. I will give it time to heal, but if it doesn't then I will have it extracted to. Cheers!

@@onewithnature8435 Then maybe you don't have anything wrong and its just a trauma that takes some time to heal. Tooth and surrounding gums are very sensitive and takes time to heal.
If possible try to avoid chewing from that tooth even a little and use your other side for now. You mentioned pain came to a manageable level is a very good news. Most probably your tooth is fine then. Just require some time to heal.
You know sometimes trauma or injury to gums also feel like tooth pain. If gums are inflamed for any reason you may feel its the tooth. Sometimes its referred pain and the patient can swear it's the tooth.
Your dentist is right. Just give it some time and give it rest. Giving it rest is very crucial for quick healing.
      Good luck. Keep me posted.

@@Kashif314 You might be right, when I slipped on the ice, fell on my back and clenched my jaw, all of my upper teeth hurt. So I wonder if the pain is coming from my jaw and radiating to my teeth. Do you plan on getting an implant?
