He mistakenly assumed that because it was a period piece she'd be wearing one leading Kidman to assume he's saying she's had to wear one to make he figure look better.

Generally actresses in period films will wear them, not cause they're fat, but because its authentic to the time.

So when an interviewer says anything challenging to them or questions their choices etc, it's like a circuit breaks in their head and their instinct is fight or flight.
