    I’ve been working night shift for 3 months now as a new grad in a LTC facility. Managing 25-31 residents a shift is hard work & I’ve learned so much about time management and my “nursing gut.” LTC nurses don’t get enough credit.

    I’ve been a CNA for almost 4 years and got just accepted for LPN school, so I appreciate what you said about CNA’s. It’s definitely not easy especially in these times, and not every nurse is like you an appreciates us. 😊

      Congrats!!! that's awesome! I encouraged the girls I worked with to go for their license. You all do so much and deserve much more. CNA's don't get treated as well as they should.

    Girl I had 40 patients, my first year as a LPN. I worked overnights,and if you get the patients who,receive pain meds out the way first. It usually helps, your shift run a little smoother. I would then, pass any other medications that were scheduled. Treatments would be done last, since they usually took some time. Having a routine that works for you,is the only thing that will help. Asking questions during report ,will help. Also working with the CNA'S,who's on your shift will help.

    Completely agree. Been a CNA the last 3 years, lots of chronic pain from it and had to be out of work and get physical therapy a few months ago. I graduate as a RN next May and plan on continuing to work in long term care.

    Before I went to nursing school, I worked as a medtech for 2 years at an AL. Depending on how short the staff is, Id have 25-45 patients and experienced the worst already. After nursing school, I got a job at a SNF and I’m telling you that I was confident right away with med passing. If you’re a caregiver, I encourage you to become a medtech first, if you don’t feel like you aren’t ready for nursing school yet

    This is one of the best videos for lpn tips I've seen. I've worked in a nursing home as a newgrad on overnight having 58 pts by myself. I did that for 5 months maybe would've done it longer. I hope new grads are taking notes!

    I like this video.
    During my Lvn clinicals, we follow a nurse and a med aide to get familiar with med pass since its a big part of the morning
    The cert med aide would take all the BPs in the morning so that helped.
    Im glad we are learning time mgmt.

My sister is an RN and I don't get help and feedback from her because she lives in So Cal and I live in Northern California but I'm grateful I was able to find your videos. You inspire me, that anything is possible for working moms. I currently work full-time so it's tough but not impossible hopefully.     I am in this situation now. I just got my RN license in the 26th of Jan and I got promoted in my agency job from CNA to RN and they work with a lot of LTC facilities and basically I was thrown in and I’m drowning. I actually went on TH-cam to find tips and I’m glad you posted. Also with the CNA with just transitioning and being a CNA a month ago I respect my aides and try to understand where they come from because I was there.

      If you got RN license get out of LTC.In hospital you will have a max of 8 patjents in med surg floor.Why you get 20 + patients.

      @@Lovebk98 because the 20+ depending on the LTC have moderate to low acuity and low turnover. med surge has high acuity and high turnover, 8 patients end up being more work.

    In the state of California you can’t use a wrist cuff regardless of what the facility says. They’re inaccurate it has to be a state approved arm band

      It's really overwhelming at first, but you will get it. Don't be afraid to ask lots of questions and ask for help. You can do it!

    Thanks for sharing! I’m a new grad RN. I’ll be starting my new position the end of this month at a Transitional Care Unit. It’s categorized within the nursing home umbrella. I currently work there as a Unit Secretary. Now I’ll be working as a RN. It’s through the new grad residency program but I’m still nervous.

    Great tips. I’d like to add to know your patients status if they are DNR or full code and keep that handy on report or in your notes. Also keep an eye on allergies there have been times I got an order and failed to see that said patient was allergic to said antibiotic, hung up the phone only having to call back wasting precious time. Other than that also know that these facilities are so extremely hard to work at but you gain so much experience and it’s worth getting that experience.

    Another tip, for patients that have alot of meds, pre pour them. You're not supposed to but on days that we are short and I have to help with care, pre pouring my 8pms does help.

    OMG I was so glad I found your video right on time for me. I just completed my LPN program here in Michigan 10/20/21. I’m look for a grad nurse position here but this video is good because during my program we couldn’t do clinicals do to Covid-19. This was very helpful to me thank you again.

      Glad it was helpful! And to be honest you don't really get a feel for working as a nurse until you get your first position and grow from there. It's definitely not like clinical so don't think you missed out on much. It is helpful but not the same. Congrats on completing your program! That's awesome!

    I agree be nice to your CNAs, been there before my LVN, and in my experience i've been treated absolutely Awful by the very LVN I had to help. Trust me, LVNs like that got stuck with NO CNA.

    Hey Justine! I’m a new grad LPN since May. I’m starting my new role as a LTC nurse Monday! Will be training for 10 days. Excited and nervous. I’ve worked in assisted living since I passed my NCLEX in May. I’m just hoping that I did not forget everything I’ve learned from school including some clinical skills. At the assisted living all nurses are able to do there is med pass and no treatments. Wish me luck! And I will take all your tips with me!!

      Did you get the 10 days promised? Because a lot of time you get only a few days because they are short staffed!

      @@katrinastevenson352 yes I did! So fast forward 2 weeks after that, they tried putting me on the unit 2nd shift with 38 patients to only one nurse: me. I went right to the DNS and told her hell no. She immediately put another nurse on with me. Isn’t that crazy?! Even she said that’s too much for one nurse let alone a new grad nurse.

    I'm a new Grad LVN and am torn about accepting an offer at a Subacute and Rehab or just waiting for a position at a Urgent care clinic. I remember going through Clinical's thinking, "I don't ever want to work at a SNF", due to the high stress environment from learning the computer systems, policy and procedures at the facility, learning patients and their needs, supervising, ensure meds are given within the time frame, etc!. But I feel like there is so much I can learn From a SNF/Subacute. I seriously Admire those Nurses at SNF, LTC, Subacute!

      SNF/LTC is not my favorite but I did learn so much and who knows how long you may wait to find a position in an urgent care. Personally, I would rather work in a SNF vs. urgent care especially during a pandemic but I am a mama of 3 and feel that you are more at risk for illness at urgent care. Everyone is different and you will learn so much in both settings. Good luck to you on whatever you choose and congratulations 🎉🍾

    I’ve been a CNA for 6 years, what pushed me to finally over step my fear and go into LVN? My daughter that is now 9 months old. Working during this pandemic has been so heavy. I truly appreciate your videos cause it’s tough being a mother and a full time worker. I’ve been having a hard time balancing life as a full time mother + worker, but your videos has seriously helped my confidence and emotional health. Thanks for the motivation & information 🙏🏼💖

    I’m a new nurse working in LTC and I’ve been so overwhelmed by my normal duties and then our skilled nurse quit and now I’m having to do med pass, skilled assessments, weekly assessments, dr appointments, dialysis, admissions, discharges, etc.. all in the same shift and I’m so overwhelmed and stressed

    I started at a rehab center yesterday. And on my first day they took me around asking someone to train me and everyone said no to us. Like literally said no to my face. I noticed that just like you said, they may just throw you on by yourself, which now I have to think if it’s the right place for me, because giving out meds without help I feel would risk the patients. Idk def scary though. Thanks for the tips!

      Ughhhh...that’s such a bad first impression. I’m sorry and yes it’s really hard to learn in your own when you are new to the field. If they don’t want to help you learn right off the bay you know if you need help they won’t be there either. I would be very uncomfortable working with people like that! I hope you find a coworker who will train you well and be there for you if you have questions or concerns because you can lose that license!

      I start in the spring in LA . Everything is online right now we are waiting on books and orientation for other stuff so far the staff has been really amazing I’m super excited for this journey

      @@sheriehammonds387 that’s great! I’m in term 5 and for the first 2 month he had clinical shut due to covid we don’t do anything anymore except on sundays but even then we can’t go tot the skills lab. You’ll do great!

    Hi, I’m a CNA and I just quit my job because I’m planning to proceed LVN and CNa is not an easy job. It’s exhausting and appreciate that you appreciate CNA’s and you don’t mind helping them coz not all Nurses are like you.

    Love your video. So what would you say is the hardest thing about working in LTC as a LPN? I’ve been a LPN for one year and have only done assisted living. I passed meds for 50 patients but didn’t have to worry about any other skills like wound care, vents, etc. I’d like to work in LTC for some experience because I eventually want to go back to school for my BSN.

      Hey thanks for watching! OMG, 50 residents!? That's insane! There are a lot of hard parts with working in LTC, but I'd have to say the amount of work I was required to get done in such a short time. I am going to make a whole video on this because I can name a lot of struggles! Don't give up on that BSN!

      Oww thats unsafe for you.No matter you work hard dont do 50 patients.The nursing home doesnt care for you if something happen .Your license is a priority.You can work as prn in skilled facility/rehab.So ,you will have wound care experiance.The wound care is not hard like we did in nursing school demonstration.

    I had 40 residents on the night shift that 6am med pass was tough.
    I hear ya tho, The CNAs are the Heart and Soul of the Nursing Homes. They should be paid the most cause they do the most.

    You talk too much and are wasting time. Get to the advice please. 90% of what you’re saying is unnecessary. Stick with the med pass, please. I am not being unkind. I think you’re doing a good thing.😀

    @heyjustine I’m starting a charge shift nurse position at assisted living as a new grad. Any tips on differences in shifts ? I haven’t decided whether to do 6am shift or 2pm…..Btw thanks for your videos they are helpful! 😭

      Hey Cindy! I did 6am because I’m a morning person and enjoyed getting out by 3:30 to enjoy my day and my kids would be out of school at 2:45 so I didn’t miss much. If you don’t have kids the 2pm shift may be more appealing because you can sleep in and get things done early in the day. You also will get a differential in pay.

    Hey justine, can i have your opinion- do you think its a good idea apply for an LVN program during this pandemic? I want to get actual hands on experience instead of online everything and not being able to do clinicals at a hospital or other healthcare setting- thank you!

      I think you should go for it! Honestly it may be easier which helps with stress. You won’t get the experience in nursing school; it all comes when you start your first job. I’d rather NOT be in the facilities right now but if you end up going for clinical just be safe! Good luck!

      Yeah, sounds about right! They are so under staffed and over worked. Most will throw you out to the wolves so to speak 😐

      Ugh, I only had 1 full day of training. Today I was supposed to orient, but they threw me on the floors. Horrible

      These are questions you ask during interviews. How many days of training do you offer, and can I request more days if necessary.

      Yes. I was an LVN for 9 years in a clinic with other RN and MDs. Ended getting stuck doing same work as RN with much lower pay then them.

    1) Don’t do your med pass😂
    2) memorize patient’s meds and just start popping 😂💀

    I advise LTC for experiance only.The nurses in LTC overworked and underpaid.A lot of paper work than direct patient care.

    I’m working on my CNA right now. I plan to get my CMA right after. I think I will get my RN at some point. I’m already 38 years old. I feel like I’m going to be pretty old (by job standards).

      A lot of places prefer nurses out age because we are known to be more loyal (stagnant) when it comes to working at a facility. So you should have no problem getting a job. I don't promote loyalty to any facility though

      38 is young! There were many older LVN students in my program and they did the best in terms of grades and punctuation! The older we get we realize we don’t have time to waste!

      3 days?? I had a week...my advice is do as much as you can do on your shift. Try not to leave dressing changes, or pharmacy refills that need to ve faxed, and don't wait to chart. Whatever you don't get to remember there are 2 other shifts as my DON would tell me. You just can't do it all.

    No disrespect ,but I highly doubt the Nurses need advice for being "nice" to the CNA's ,its more like they should be nice to the Nurses. Its always a few with very NASTY attitudes ,no matter how nice,kind ,courteous and respectful you may appear. It can be stressful and sickening at the same time .

      I agree, it goes both ways but from my personal experience I’ve watched my nursing coworkers be straight up lazy and make their CNAs do extra work that the nurse could have or should have at least helped with. CNAs do a lot and us nurses would NOT want their job! But you give to get respect and yes some are just rude no matter what but that’s all positions.

      I’ve watched nurses refuse to help change patients just because they didn’t want to. There’s a lot of laziness in the nursing home and it’s sad because the patients are the ones who suffer.
