FunkyAlanzokaCat He wrote the majority. Portnoy did put his input on some parts that weren't 100% written quite yet. Listen to some of the demos with The Rev playing. Though almost identical there are differences. He left most of it the same though. I believe there are a couple of songs on Nightmare that Portnoy had to write a bit more of the parts than most of the songs.
