The blocks at the end of the stage were incase you got through with a second power up; the two doors led to the yellow blocks at the very beginning of the stage to allow you to take damage.
He was talking about the desert bus level. It's still there, but he can't play it since he didn't write down the code and you have to play a level to be able to go back to it later.

@Ryukahr The front appendages on a spider are called pedipalps. They are not legs or arms, but specialized mouthparts used for holding prey while injecting venom. They would not utilize pants. They would have something more like a glove/bib wrapped around them.

Time resets every time a screen changes, such as going in a pipe, or a door. So he was at 1 to 0 when it said 0 through those changes.
