How does the excessive amount of money foreseen by the quantity theory of money get into peoples' hands? How does that money get into circulation? Does the government give the money to banks for lending or are there other mechanisms for getting excess printed money into circulation in a way that contributes to inflation?

I have a questoin about demand pull and cost push inflation. Wouldn't that only affect a certain group of goods and services, such as toys and gadgets. But for inflation to occur, doesn't the general price level have to increase?

Why didn't we get inflation during the bailout of 2009? Isn't the printing of money supposed to cause inflation? If it didn't cause inflation, then why don't we slowly print away our debts?
