Clinical History:
Around 50 years old female patient came with:
Menorrhagia
Dysmenorrhoea
Ultrasound Findings:
Bulky uterus
An oval hypoechoic intramural fibroid at the anterior myometrium
Thickened endometrium (15mm)
Intramural fibroids usually shouldn't cause a menstrual disturbance.
So, in such cases, we try to evaluate the endometrium carefully.
It's better to magnify the image using the 'Zoom' & 'Depth' buttons and search carefully.
In this case, the patient also has a tiny submucous fibroid protruding to the endometrium from the myometrium and that's the principal factor causing a menstrual disturbance.
TVS & Pelvic MRI are definitely better options that you may not get available everywhere.
