I found as an ER Dr the easiest way to Dx Bells Palsy when findings were VERY subtle and forehead not affected (yet) that the most reliable way was to watch the eyes blink. the affected eye does NOT fully close with blinking = Bells.

@@mamunurrashid3141 it is little confusing and wrong diagram 2 and 4 are upper motor neuron lesions so both eyebrows should be up and 2nd is left side UMNL , because UMNL is always contralateral so here both eyebrows should be up and not able to smile on right side

We should remember the difference in the symptoms of UMN and LMN
1. Left lower motor neuron lession due to absence of nasolabial fold and unable to raise his eyebrow
2. Right upper motor neuron lesson coz only absence of nasolabial fold but pt can raise his both eyebrows
3. Right lower motor neuron lesson (vice versa of 1)
4. Left upper motor neuron lesson vice ( vice verse of 2)

Shouldn’t option 2 and 4 be switched as the sign is always named by the side of the sign and not the side of the lesion?

Shouldn't the UMN examples be half a face is fully expressing, but the affected side has eyebrow up too. so it should be 2 eyebrows up on the UMN cases.
? This is a trick bc in all UMN lesions the other UMN can innervate the brow, so ALL 'intact' sides of the face should be able to raise eyebrow. 2+4 have a dead eyebrow on unaffected side, but the un-lesioned UMN should help raise it, right? Wasn't that the point of the differential.
1. left side is fully numb, with right eyebrow up so UMN is intact, so this is LMN on ipsilateral side (left)
2. right side of face fully numb signaling LMN on R, but with upper of left numb too. why is upper of left face numb? Why can't left ipsilat UMN innervate it.
3. right side of face fully numb, so LMN on ipsilateral R side
4. left side of face fully numb showing LMN on left, but upper side of right is numb. The right side has a lesion not discussed in this video (only numb upper right side of face).

2 and 4 are UMNL bcz upper motor has the privilege to give ipsilateral and contralateral suppliment to the face of the upper region. 1 and 3 are LMNL because the eyebrows are affected due to the lesion of the motor in the whole side face.

@@the_scrunchies_studio6952 it would be umnl only as patient is asked to raise their eyebrows and the only way we can check that is by comparing the normal side . Since affected side of patient is right side of face with the right eyebrow being raised as much as if not more than the normal side means that right upper half of face is intact therefore the lesion is left umnl (as umnl is contra lateral to affected side).
