It doesn't. I said most common cause of AR for USMLE is aortic dissection, where there's aortic root dilatation secondary to retrograde propagation toward the aortic root. In this patient, the incompetence of the prosthetic valve is causing AR, as evidenced by the massive pulse pressure (diastolic = palpable = "zero"). Prosthetic valves can cause shearing forces on RBCs leading to schistocytosis.
