@Dr Shashank Akerkar Sir,I am from Bangladesh. Last year i consulted with u.I am A S patient. Now i am on Bangladeshi doctors treatment. Sir problem is i don’t go to bed at night due to pain & pain increases in the morning. Sir after taking clonazepam. 5mg i am feeling good but my rheumatologist Don't allow it. Sir can i continue clonazepam 5mg for continue though u prescribed me etizolam .5 mg but it is no available in Bangladesh. Thanks Sir.
You can take clonazepam. However it would be better to manage sleep without the medicines. Try & reduce stress if that is the cause. Go for a short stroll at night before going to bed. Avoid mobiles, TV before going to bed.

PRP has not been routinely used/ recommended form of therapy for AS.

Biologics ki e tarah jak inhibitors available hai. They are originally a medication for other type of athritis. Only pfizer used to make it, now there are different companies making it. It is named as Toficinitib. Google it. They have now promising results for AS also.
My blood group A+ , my age is 29 , doctor said i hv spondyloartharitis , but my all blood group normal, x ray mri is looking normal .....i hv probelm past 8. Years ....is that true i hv spondyloartharits ?

I wanted to ask that is it possible to have sacroiliac inflammation as shown on MRI (sacroilitis) but no axial Spondylitis/ankylosing spondylitis? I did not had any accident or trauma but i have been in pain since 2018 starting. Blood reports all normal, HLA negative. No morning stiffness but consistent pain and discomfort. Muscle spasms 3-4 times in last 3 years.

This can happen in athletes. One can get minimal edema in sacroiliac joints due to sports activity/ exertion/ running marathon. This type of edema generally affects a particular part of the sacroiliac joints. However this would remain for a short duration & unlikely to last for 2 years. One needs to examine you & have a look at the MRI plates. Do consult a rheumatologist & discuss for a definite answer.
Sir does sacroilitis could occur due to prolonged sitting routine and bad posture or is it always associated with ankylosing spondylitis

Thanx a lot, very well explained !! 👏🏻I have axial SpA , diagnosed very late , one of hip joint is little fused but other than that all other joints are ok by far . My question is , if I’m on biologics can it spread to other joints ? Or it will stay as it is for life long ?
U have to take biologics life long or as per ur doctors advise.there is a time limit of every biologics like 1 or 2 month . After that the control of biologics will be over then again u will need biologics. Say lifelong no end. Saaz dr ,ds , saazo delays the risk of fusion but doesn't cure as. U have to be active alert n sensitive towards ur body n particularly to ur joints n try to always check the mobility of ur joints.if u feel any difficulty in the mobility pls do an x-ray check up.n follow the remedies
With biologics the chance of it worsening or affecting the other joints is less.
@@DrShashankAkerkar thanx a lot sir , actually I hv started feeling stiffness in other hip joint too . I’m on biologics , but can’t do regular exercises or can’t take my NSAIDS. Because of some reason ! That’s why I’m concern about it .

My doctor suggested Biologics(Adalimumab 40 mg) for 3 months, once in two weeks for AS problem. Do I need to continue Adalimumab after 3 months? What's your view Sir?
Yes. Taking it only for three months may not be sufficient. Please talk to your rheumatologist in details about the same.
@@DrShashankAkerkar One thing surprises me. Every rheumatologist strongly suggests for regular physical exercise for AS. But my rheumatologist(Dr. V.V.Kaushik,Apollo Chennai) told me not to do physical exercise because he said" Bone me sujan ho gya, kabhi bhi exercise nahi karna". What do you say Sir?

Dr.sir good afternoon.Thankyou for guiding SS pt.sir I m ankylosing spondylitis patient.i m taking adalimumab 40 mg every 2 week from last 4 months but now my problems increased my crp and est increased which were normal from 4 months. Kindly help me what to do.i have also used injection 2 days before.but no improvement even I m taking saaz 1 gm continue. Very acute pain and swelling in my hip and ankle pain in chest ribbs.chest xray is normal.
Aviod any kind of injection go to ayurvedic treatment and do exercise daily

Sir I have started sulfaselazine one month before but only 5% relief. In my hip pain I met rheumatologist he suggested me biologicals as I have active bilateral sacrolitis....AXIAL SPA. he said any medicine will not work for me except biologicals.sir I afraid side effects of biological and want to try atleast one more month saaz.will this work or not in my condition.i have not in serious condition.sir pls pls guide me🙏🙏
You can always try Saaz for a few months. Exercise regularly if not doing so. However do have a 'stop loss order' What this means is that have a fixed time limit eg 3 months beyond which if there is no relief you should discuss with your rheumatologist & go ahead with biologics. Biologics are quite safe if taken under supervision & with proper precautions.

@@Beamlight7 yes I have ankylosing spondylitis and hla b27 positive since 2014.it was 70 % cute with exercise and panchkarma , ayurvedic treatment but when I came in chennai one year back it started again .may be because of weather.i have started sulfaselazine in January 2021,after 3 months I have got 60-70% relief.somtime pain increases.i have noticed that my problem increase with weather conditions but it not happens with all🙏

Actually 2 weeks back I started saaz ds, and was sufferring with sacroiliitis since 2yrs, but the different orthopedics I went to, gave meds which didn't subside th issue. The last ortho I went to did my MRI and thats the sacroillities was diagonised. He gave meds which didn't help. Now I am on rheumatologist treatment. I am Hla b27 negative, but don't know what went wrong at age of 30, now I m 33. Ironicqlly I was a vegetarian, eating fish and rice too, but no milk from age 20 to 30. Then I started non veg again, and ate lots of sweets, and processed meats, that when all symtoms started.... Are u still continuing with ayurveda ??? I wanna give ayurveda a try, I believe ayurveda can help a lot simultaneously... also did u try biologics ?

We already have quite a few good effective medicines available as of now. There are a few like Tofacitinib in the trial phase as well.
