I think those spikes on the second offset might be from extra nodes inside the first offset from the overlapping areas that closed the negative space. In older versions of Inkscape you could get rid of them by breaking apart the object, then perform a union. I can't remember the workflow on this version. You could also use the select node tool to select and delete them.
