Even harsher than the game over screen in Splatterhouse 3 is the changes that happen in the story if you don't complete certain levels within the time limit. Having to finish the game with a guaranteed tragic ending is quite the downer. The Death Duel game over screen is downright brutal, but it fits in with the time period in which it was released because games that talk shit to the player were definitely part of the whole 'tude era in gaming.

Amazing game, the art the sound effects, the music matched all so well. A game who jump scared me back then was Nosferatu from snes, the first level was so damn well designed. The atmosphere, the art. The first time i played it was late at night and was tired, i was learning the controls and failed many times then i noticed something different in the game over screen... i got nearly an heart attack, i wasnt sleepy anymore that day...
Alien 3- The fact that timer can give you a gameover meant *the facility got overrun by Xenos, who dragged you A## away to their hive* . Worst fate of all.
I've had that happen to me in the final stage as I was tracking down the last prisoner, like maybe another two seconds more on the clock I could have reached the prisoner. All that effort, and one prisoner earned me an immediate game over.

I really admire genesis games a lot for their production quality, even with a certain hardware limitation on the color palette. Often its capacity is exceeded by creating true graphic feats in a relatively modest system compared to other 4th generation.
I agree that limitations and different graphical styles are more memorable. Take TMNT 1 arcades intro vs TMNT Shredders Revenge intro. One uses a variety of styles in its graphics and the other is just animated very well.
Alien 3's Game Over screen scarred me. The fact that I would often fail, or die, or fail to save the prisoners in time, makes me recorded this episode.
I know you had a limit of 20, but if I remember right the Game Over screen of Risky Woods was pretty creepy. The King of Monsters 2 game over screen also scarred me as a kid.
