It means, we should avoid using any word between the noun or pronoun and the relative pronoun. Thus we can use 'yesterday' in the beginning of the sentence though there is no such a strict rule.

I don't think there is any grammatical problem with the sentence but normally we use past perfect tense followed by past indefinite tense.
