Whenever i evaluate a neonate after birth, most common question asked by attendents is "sir please recommend some formula" as mother is unable to breastfeed because of cessarian.

One issue is to prime mother antenatally and even consider starting expressing before the delivery (especially if planned CS)-avoid sedating medications and use spinal anaesthesia as far as possible. Sir, Is anything like breast milk will get reduced by evening 6 or 9pm ? I personally felt , she is breast feeding till evening , after that she is wanting milk ,even after giving for 20 min or more in each breast. As I am feeding from morning , feeling some soreness( but no nipple cracking ) in breast . I am giving 60 ml formula at that time.( I tapered formula Sir, during 2 nd and 3 rd week sometimes I used to give 60ml , four times a day ,as she had weight loss from (3.4 kg at birth to 3kg), breast feeding jaundice and polycythemia due to dehydration( she was also admitted and given phototherapy and iv fluids) and at that time I also did not understand how to properly latch her and how to use electric pump.
Not that the milk reduces but baby gets colicky at that time. Avoid overfeeding, actual intake varies between feeds, 10-20 minutes feed every 2-4 hours is adequate
Each position has its advantages and disadvantages, and it is good to be familiar with 2-3 as it can help avoid bad posture, arm pain in mother as well.

Very nicely expalined in simple way... Specially about empty factor... And i usually force our nursing staff for night time breastfeeding ( more prolactin production)... So sir as we know that baby has store for 2-3 days for energy and breatmilk is verey low in volume so should we continue NNS 2 hourly and give lactogen for hunger of child and should taper lactogen according to breastmilk production?? And my question is... Sir we know that baby loses around 10-12 % of weight in initial 7 days...can we have this percentage according to daywise... Because sometime baby loses 5-6 percent weight in initial 1-2 days then we should wait till baby loses more than 10 % or we should evaluate for the cause???

Because the normal baby has stores and can use alternative fuel, we would only suggest formula top up in few situations, better to avoid unless risk factor for hypoglycaemia, mother unable to feed well etc. Even in these situations, we should ensure we keep the latching and expressing ongoing so this top up is for minimum period. Avoid bottle, use syringe or cup feeds
As for weight loss, 5% on day one is not uncommon, especially if mother received IV fluids in labour (particularly hypotonic fluids). After that, it should slow down to 2-3% per day. You are right, we should monitor weight and if weight loss trend suggests excess weight loss at day 2 or so, support lactation aggressively so we can avoid the over 10-12% weight loss situation

Get an electric pump and make sure the seal is good, and settings are ok. Don’t be tense, do more skin to skin care, direct feeds and then express every 3-4 hours without fail. Should improve in 2-3 days
Hello sir I am exclusively breastfeeding my 1.5 month old daughter. But still she is not gaining weight. At birth she was 2,9 kg. But last Friday we checked her weight. And it was 2.7 kg. Means the weight reduced. I am so tensed. She is taking feed after every 1 and half hourly. Shepees 10 to 12 times a day also passes the stool everyday. At night she sleeps well for about 4 hours continuously. Pls suggest what should I do gain her weight in a right way.
Hi, it is not normal to have such low weight gain. Have you noted the trend in past weeks? Maybe birth weight was wrong? You need to express and top up and if not having around 40-60 ml at least when expressing with pump each time, you might need formula feeds in addition for a week or so while you work on increasing output. Just feed on breast for a few minutes each time during this transition but you can breastfeed fully again once milk output good and weight gain better. Do update.
