Whether I can filled the form manually using pen or shall we printed all details using computer and printer.
Moreover in my form filled with my Doctor incharge there is no nursing manager such position so is there any issue for that

Your name should tally with all your documents .. where there's Discrepancies, do affidavit

When you are uploading, you will be required to input a completion date, so avoid discrepancy, write a future date and attach the letter

Please in my case i have been in the same unit since i was posted. From staff nurse and I'm now senior staff nurse. Do i have to use my current rank or start from when i was a staff nurse, write the job descriptions of a staff nurse before moving to my current position

The institutional email is required for the organisation's email, whereas the signitory's email is the Nurse manager's personal email
