Keep in mind, this video is about a safety issue, NOT about a bolt and its direction of rotation! Yes it may tighten but as I said in the video, there are other forces at work! These do loosen and already people have written in about their experiences with blades coming off!
Check your torque specs, clean the mounting area, make sure things are correct! This is the business end!
    Stay Safe!

I just dealt with this exact same situation on this exact adapter. Fortunately in my case, the customer understood after I explained what could happen and they agreed to have it replaced. This must be a common failure with this style of adapter because I’ve seen a LOT of them failed in this manner.

These blade adapters are very sensitive to torqueing. They must be torqued to 38-40 ft lbs (mine is a Honda powered Husqvarna) . or the blade will wander around and shear or wear off the tabs. First time I didn't torque it and the adapter lasted about 20 minutes. Afterward and another adapter torqued correctly it is now into the 3rd season.

The original blade was probably replaced with an incorrect blade without the correct star pattern, wearing down the star. I respectfully disagree that it would spin the bolt out since the blade direction only serves to tighten it. I bought a 48” rear mount Woods mower made for a compact diesel tractor with a counter clockwise PTO to replace a similar mower that was made for a clockwise PTO. Couldn’t keep the blades on it until I took the spindles off the old deck and put them on the newer one. The old deck spindles never lost the bolts and they were right hand threaded. The newer incorrect ones were left hand threaded.

Your demonstration showed that the bolt would tighten when struck, not loosen

Yea, for a moment, then what? If that blade spins up and is loose, it could spin the bolt out if it grabs it. Usually the blades spin, I've had that happen and so have other mechanics!
Either way, its crazy to grind that star off!
Keep in mind, there is vibrational and back on forth motions or harmonics that occur, acceleration and deceleration forces.
When I start getting that technical, I loose people!
Its a safety type video, not a techy vid lol,,shhhhh don't say anything lol
