    Outstanding story and explanation of IPL and other treatments! I have Ocular Rosacea and MGD and have been educating myself on relief of dry eyes that I have had my entire adult life. I am now 64.

    I have had ocular rosacea since I was in high school but was not diagnosed until I just started optometry school!! Thank you for posting this, I wish ocular rosacea was discussed more.

    Currently fighting my battle with ocular rosacea and dry eye. After seeing 5 eye doctors and doing some research I found one who listened more than the others. I’m taking an oral antibiotic and using artificial tears daily, but I do wish there was a more I could do for a clearer answer and more confident treatment plan. It’s been just over a year and I have had constant styes and 4 chalazions in that time. My vision in one eye regressed to almost legally blind at one time. It’s so frustrating.

    I wish I didn’t live all the way across the country in Atlanta because I’d love to meet you! I had LASIK surgery almost 2 years ago and ever since I’ve had dry eye. About a year ago I was diagnosed with Blepharitis and then I started having flare ups around my eyes. I still have them to this day and it’s truly depressing not having real relief. My eyes consume so much of every single day. I’m self conscious of how they look at all times. They feel tired and they’re always red. I’ve seen a dermatologist, multiple eye doctors, a dry eye specialist and an Allergist. The dry eye specialist started me on restasis and several vitamin supplements. She said the inflammation around the eyes looked like Eczema and referred me to an Allergist. The allergist said it’s not Eczema. It’s beyond frustrating trying to get some relief but this video gives me hope. I will definitely be looking into IPL. Thanks for this video!!!

    Im 14 rn but i started experiencing dry eye problems when i was 12. It didnt start of hurting but they where red and people noticed but i didnt cause they never hurted, it wasnt till covid happened and we had to wear masks that the problems really started kicking in especially when the air u breath in cames back into ur eyes. And rn as i send this comment im still facing it rn.

      The warm, dry air that comes up from the mask can exacerbate dry eyes. Now that we are not wearing masks hopefully your symptoms have decreased.

    I am 26 and I think I have that, my eyes are really inflamed and drops every hour are not helping at all, sadly I am not from the US but I am going to get the same tests soon

    I barely have any glands left. I was told by one optometrist to "just keep putting eye drops in"
    I was already doing that and it didn't help. So, I went to a dry eye specialist recently and he scanned my eyes and I found out that I have just a few healthy glands left, which is a complete shock to me. I honestly don't know what to do from here or if there is anything that I can do.

      @@ayush3334 That could be it. I started wearing contacts when I was 13 and I don't think I was mature enough to have them meaning I didn't clean them as regularly as I should. And then the past two years i've been a software engineer so I stare at a computer for at least 40 hours a week

    i went to my optometrist and he said that IPL wasn't a good option for me because he was afraid that it can blotch or cause discoloration around my eyes due to my dark skin

      Wow happy there is hope. The eye pain from clogged glands make it hard for me to paint. Their doesn’t seem to be any doctors in Ohio who specialize in this. My eye doctor just wants to slice them open and drain them. Surely there is more I can do besides warm compress and having my eyes cut up?

    I have had several IPL treatments and I have found that it does not last. It is not covered by insurance so it gets very costly for a short temporary relief.

    Was told I have metbonian glans disorder a couple of weeks ago been suffering for months with stinging and dry eyes doing the warm compress and massage have drops to done nothing to be honest infact they feel more uncomfortable really feels disheartening, from being at school remember my eyes was red and a little sore only had one appointment with eye doctor have to wait months for another one feel there's no hope will continue with compress and massage in the meantime 😣

    I have severe MGD. I have no glads visible on the photos my eye Dr took. She wants me to do the laser therapy as well as heat mask therapy and lid massage at home. My question is, if the glands are gone, what will these therapies accomplish?

      Oh no, I have been investigating and if the glands are gone these treatments can't really do much, try some drops with oils added I guess that can help, but yeah I am going to get the images soon too but I am really afraid that I also lost all my glands and the tears with oils are not sold in my country, hang in there my friend and let me know how are you doing

    👏 good doctor. It’s sad my doctor never offered to drain it. But admitted they were super oily. Gave me eye steroid for a week and told me to warm compresses.

    I have problems by my eyes aswell, if i drive or watching tv or computer my eyes turns red and burnfull the full day, i already tried eye tiers warm compress, a glasses for screens, taking breaks doing the 20-20-20 rule

      It’s important to take the guess work out by doing meibiography to evaluate the health of your tear making glands. You can schedule an appointment with me if you were in Los Angeles, if you’re not then call eye 👁 doctors to see who has the ability to physically see inside your tear making glands. Once they have evaluated your glands and tears, a treatment plan can be made.

    I have same as him. And for me it’s worse because my eyelid always double volume due to the accumulation of oil. So at the end of the day, my eyes are very puffy

    my eye doctor says that i have meibonin gland dysfunction, but my eyes never feel dry, i do get excess thick, stringy eye mucus and it does blur my vision but never dry.

    These little bumps on the edge of my eyelid right up against the eyeball are driving me up the wall. This only started happening for me last year but I have them all the time now on any spot along the edge of my eyelids and a lot of the time it bothers me the whole day.

    Hey Doctor I been dealing with with this issues for over a year now and feel like just like Piero!, I’m from Los Angeles and seen 3 optometrists and 2 ophthalmologists already, they barely diagnosed me with MGD I’m on a 90 day warm compresses, Doxycycline, Refresh, Xxidria and eyelid spray cleaning treatment plan. If this doesn’t work out how can I set up an appointment because I really feel I’m in time to heavily impact MGD.

      Atrophy can be a result of prolonged inflammation when the glands are clogged for so long. I cannot give you medical advice on this channel.

    Is it normal to have no results after first treatment? I had my first IPL for clogged glands and inflammation and redness - no changes. I have 2 more sessions scheduled with more strength but I’m feeling discouraged especially aft hearing this story...🙁

      Meibomian Gland dysfnction takes a while to develop, and take several treatments as well. It’s not an overnight treatment

    I’ve got dry eye ever since my PRK treatment and my surgeon has written me off but not knowing how to fix me. Went to an optometrist and she mentioned many of the same things in this video and recommends IPL among other topical treatments. What are the side effects of IPL short and long term?

    What do you mean with „over the lid shield“? It’s at around 7:25.
    I have the same simptoms and guess it‘s also because of inflamation. Have you done the IPL sessions different as „normal“? Maybe more close to the eyelid? How is this done for the upper lid? I just want to make sure to tell what you have done here to my therapist (live near Munich in Germany so can‘t really come to you ;)

      There are two types of shields. One of them sets on top of the eyelids when they’re closed. The other one sits underneath the eyelids were on the cornea. It’s like a big contact lens. This allows me to get closer to the Meibomian glands for more effective treatment.

      Do you need more support if you are spending many hours in front of that screen. Working on a digital screen, decreases your plane, crate so that the chairs are not being spread over the eyes to moisturize them as you stare at the screen. When the oil heaters are not being blinked away the second and end up, clogging that your plans, which eventually leads to MGD.

    Dr. Brisco, for long term maintenance treatment of ocular rosacea, is it normal to be on low dose steroid (.2%) eye drops indefinitely? I'm concerned about skin thinning and the chronic problems that can come with it.

      I like IPL because it is a drug free and more natural way to treat ocular rosacea and dry eyes as well as meibomianglanddysfunction. We use light to treat instead of pharmaceuticals.

There are a number of different homeopathic remedies for dry eyes, meibomian gland dysfunction, red eyes, tired eyes etc. Because there are several thousand different homeopathic remedies, you have to match it with the patient’s history, their personality, and how they have responded treatments in the past. There is no one treatment that works for everyone.

    They always prescribed lubricant eyedrops and gel, and it is almost a year still I'm not feeling better

    I have a similar issue, dry eyes, repetitive chalezions and styes, infections, and inflammation. My doctor has put me on Doxycycline HYC to control the inflammation. Is this a good treatment option compared to your IPL treatment? Or should I mention this treatment to my doctor, and/or find a doctor that offers this treatment ? Thanks

      Ask your doctor about ways to control inflammation without having to keep taking antibiotics. IPL is drug free

    I love lectures but case examples even better.
    I’m doing everything I can to halt and reverse my dry eye conditions because I’ve had the condition for almost 25 years. I now realize from this video not to let inflammation become chronic and do even more damage over time. Thank you for that.
    I’ve found improvements using MSM eye drops. Also eliminated all seed oils from my diet because MD is in my future.
    I believe Dr Chris Knobby is correct about seed oils being root cause of diseases. And it’s easy to remove from my diet.

      Go to that giant online retailer and read the reviews for a broader firsthand usage.
      MSM drops helped my eyes heal from uveitis on the surface of my eyes, but I did get diminishing returns after while. I am grateful for the healing and continue to use it. I have recently gotten over a ringworm infection which is a yeast infection, and I am once again seeing improvement. Eye disease can be related to inflammation.
      I also use other eye drops and gels such as by Simlasan. And I use Astaxanthin, Zena xanthan, lutein and other vitamins because I don’t absorb nutrients well.

    What’s the difference between MGD and Ocular Rosacea? My doctor diagnosed me with MGD but I’m suspecting ocular rosacea as well... Another question; would you advice to do Lipiflow or IPL if meibomium glands are blocked and inflamed?

      My booming land dysfunction is a constellation of issues which can include ocular rosacea. Ocular rosacea are the tiny blood vessels called telangiectasia which feed the inflammation. This can also lead to facial rosacea. Both treatments are effective. You would have to ask the Doctor Who physically examines you what is recommended in your particular case.

    Thanks for this video. Like Piero I was a teenager and diagnosed with "the worst case of blepharitis and dry eye" the opthamolagist had seen. I still use cotton swabs and baby shampoo to clean my eyelids daily as they develop crust if I don't do this. It is difficult to do it consistently as well. I will research into IPL and what it entails. I have recently started to develop meibomoid cysts possibly? Which is the hard lump on the top eyelid and I am concerned this will develop into something worse

    Hi Dr, I am experiencing the same symptoms as Piero and I plan on doing the ipl treatment for the first time next week. My ophthalmologist said that I have blepharitis and clogged meibomian glands. Do you think this is the right treatment for me? I have a history with keratitis and gpc aswell.

    If your oil layer of tear is blocked or your oil/lipid quality in tears is not proper ,it is detected by schimers test ,doctor performed my schimers test and it read 25 and above in 3 mins .he told you have enough tears no need to chech for oil/lipid is he correct?,does these defect causes you to wear spectacles or your vision gets poor?.pls reply

      The Schirmers test measures the aqueous layer of tears rather than the oily or moisturizing layer. Vision can fluctuate when the oily layer of tears breaks up.

      Most patients need to both melt to unclog the blocked tear glands, then use IPL to treat the inflammation. Treatments need to be repeated annually for maintenance like having your teeth cleaned. Yes! It works! I’ve helped thousands of patients

    I did ipl treatment for dry eyes 3 times
    I got relief to some extent as i using hyne eyedrop and lotepred eye drop. i am also warming eyes everyday by using warm cloth on eyes.
    But when i did schimmer test to check if any tear production increased but i surprised by knowing that tear production has not increased i.e at 5mm on schmer test strip.
    I did that because my eye drop using frequency not decrease i.e 7 to 8 times a day even after ipl 3 times.
    Please what to do
    Is any best treatment available?

And then, How can we do the maintenance? Once a year? In my country the sessions are Just TOO expensive and I'm afraid of It Just doesnt work. My problem is the same as Pietro, since I was a Kid!       The treatment depends on the type and level of your condition. I can tell you that you need 2-4 weeks between IPL treatments.

    I couldn't understand the scrub treatment he is doing at home (due to masks). I have Ocular Rosacea, Blepharitis Meibomitis, Disorder of lacrimal glands,Squamous blepharitis, Collarettes. Having 3rd IPL next week. Improvement seems temporary each treatment to date. I was wanting to add BlephEX scrub office treatment. I am very uncomfortable at times, please any suggestions? I use TeaTree daily, EyeEco heated gel pads daily too. I take warm Qtips several times a day with TeaTree foam and clean lash line also. Crusty issue is better but still I have inflammation and my right eye is sore most days. I am a redhead fyi. So facial rosacea is present at times. Please any suggestions? I also read Tarsus Pharm is coming out with a "cure" for mites? Is this your understanding?
