Mavis explained it at the end. Yes, Zeref truly fell for Mavis but Mavis still hesitated to love him with all her heart. That’s why only Zeref’s curse affected her. Zeref also knew that fact and his face showed that he suffered for it - to love someone with all his heart but knowing she didn’t really love him back. I guess that’s why he felt a little bit of uncomfortableness every time he knew Mavis was around him. I thought he truly wished to get her full love. Sadly the moment they opened their hearts is the moment they gave the other the death sentence.
