  • Time to update some of the information from one of our first episodes on PLAGIOCEPHALY with ‪@AskDrChristian‬ !
    That was a long time ago, and so much has come out since then. Here's a bit of an update on some of the data. A lot of the mechanisms are still similar, but we have a much better understanding of the long-term outcomes with plagiocephaly.

    Also adult here 31 went to a craniofacial surgeon to get scans and he said it wasnt to bad. Opened the scans myself and my face is deviated by 7° degrees. Doesnt sound like a lot but my face is 2cm off center with an obvious twist to the left. Now im in therapy because i also lost a good chunk of my hair making it quite noticeable and even shocking to look at. I still dont get why so many doctors pretend that its nothing to worry about. Mental health is very important. Hope they figure something out in the future for adults. Thank you for bringing awareness ❤️

    I'm an adult with Plagiocephaly, I only found out recently. All those years of taunts, teasing, and abuse by people because of my head deformity. Even now in 2023. It's so depressing, and humiliating. Plus I have a rare genetic disease, Tyrosinemia Type 2.

      Hi Tudor, and thanks for reaching out.
      First up, I find it appalling that people can be so mean to each other over something you have very little control over. I'm sorry to hear that you have had such a rough path growing up. I wish I could provide you with some direction as an adult, but I just don't know what is best for the grown-ups - however a few people have commented with suggestions as well.
      Regarding Tyrosinemia, I have not heard of this particular condition, but after some short reading it certainly sounds...interesting. Have you had any discussions regarding the guts' role in being able to assist in the breakdown of tyrosine? Certainly some interesting pathways that could be utilised there!

    I’m 21 years old and my whole life I’ve wondered why my face was so asymmetrical it was like a different face on both sides. I would constantly take pictures of the bad side and never understood why it looked like that. I love the other side of my face but was never able to fully embrace it because I’m so hyper focused on the bad side. Now I finally put the puzzle pieces together and remembered my family talking about how I used to always sleep one side of my face. I wish my parents didn’t try and hide this and would’ve been honest. Now I realize almost all of my insecurities about my face trace back to this. I’m so frustrated and have a list of surgeries I’m thinking off. They’re terribly expensive but I’m adamant because I want to see what I would like if I was more symmetrical. Im tired of feeling ugly. People have always commented that my face is long and skinny but they never realised it’s really all because of that one side that stands out more. I’m afraid the surgeries might change the good side of my face. The main surgery i want to get is a double jaw surgery do you think this can help fix the unevenness and push out the flattened jaw more?
    I was just ranting sorry but here r my main questions please
    -does sleeping on one side as a baby also effect the body?? Because I swear I also see an unevenness in the shape of my body where my hip and chest is noticeably flatter
    -is it true that it effects the nose and makes it more “beaked” like bc one side of my nose looks normal and fits my face meanwhile the other bad side is literally like a beak it protrudes more and my nose is super slanted.
    -is it also true that it effects the eye because on the bad side my eye appears to be bulging from the side
    Then of course my whole cheek and jaw is elongated and flat
    -I too have always noticed that I can turn my head on the good side pretty well but when I try to look over my shoulder on the bad side it’s not as comfortable and I have to shift my body more. I can’t believe all these things that I thought were random all connect back to plagiocephaly. It’s so annoying knowing that it was easily preventable.
    -does it affect cognitive functioning? Brain/mental health? Tbh I’ve grown up struggling a lot with mental health could this have played a part? My head is so flat on one side that I sometimes wonder if there’s enough room for my brain 😅😭
    -as far as neurodevelopment delays is this actually backed up enough? I’m really freaking out with you saying plagiocephaly can affect quality of life all togetehr. I’m freaking out bc I thought this was just a cosmetic issue is this something that can actually have that big of an effect?? Because my family always talked about how as a baby I didn’t smile much or laugh much maybe at times but even looking at videos of me as a baby I had a resting face no matter what was going on around me and would laugh and smile just a bit here and there but always seemed to be in a bad mood lol later on as a toddler I became super fine though emoticons but I have always struggled in school. If you could please direct me to a book or article that goes deeper into this I’d appreciate it
    Thanks for responding if you do I’m really struggling with this rn lol

    • @AskDrChristian
      @AskDrChristian  2 หลายเดือนก่อน

      Wow now thats a response and a half! I'll do my best to cover what you are asking.
      1. "I wish my parents didn’t try and hide this and would’ve been honest."
      To be fair; they may not have known. A lot of parents get told (rightfully) that most babies grow out of this. The problem is when they don't, but with our current knowledge it is difficult to know which kids will and which kids won't.
      2. "The main surgery i want to get is a double jaw surgery do you think this can help fix the unevenness and push out the flattened jaw more?"
      I have no idea! This falls into the realm of cosmetic surgery, and I am very unfamiliar with what gets done there.
      3. "does sleeping on one side as a baby also effect the body?"
      yes it can; we see this as an association with plagiocephaly, hip issues, and foot issues. If there is a general torque to your body going on, other areas may be involved.
      4. "is it true that it effects the nose and makes it more “beaked”"
      Not sure on this one. I think genetics comes into play a bit more here.
      5. "is it also true that it effects the eye"
      Yes, if it has progressed enough into the face it will affect the orbit and subsequently the eyeball as well.
      6. "does it affect cognitive functioning? Brain/mental health? Tbh I’ve grown up struggling a lot with mental health could this have played a part?"
      Research suggests that it may be ASSOCIATED with cognition issues, but not CAUSE them. It's like saying the bruise on your leg is associated with your discomfort, but it was the bad tackle that caused it. One theory is that because the upper neck is one of the largest sources of stimulation for our developing brain, when it is limited in movement, the stimulation is limited as well. This area needs further investigation to really nut out finer detail.
      As for articles, have a look at this document. Its a little old now (and due for a refresh), but it contains a good chunk of what you might be looking for.

      As to what can be done for an adult, I'm not so sure. I feel like getting surgery to improve something like this really comes down to how much it is impacting your daily life experiences. If it is interfering greatly, then I'd be talking to specialists to see what can be done for you.

    I'm confused about the information regarding helmet therapy. My son is nearly finished with his helmet therapy and has a full correction of plagiocephaly. Would the impact of having phlagiocephaly for the first 16 months of his life still impact his quality of life long term? And is there still an increase of developmental delays. His plagiocephaly was caused by torticollis (which is currently fully treated through PT).

    • @AskDrChristian
      @AskDrChristian  ปีที่แล้ว +1

      Hi there Christine,
      First up, sorry if I have created some confusion. Let me try to clear this one up.
      Helmet therapy is useful for head shape, but the research is conflicted in two aspects; firstly, a very recent systematic review (the highest level of evidence) showed that it improves the shape quicker, but over the long term it is not that much different to natural history. Secondly, there is limited evidence to show improvements in development over natural history either.
      The good news though is that PT, by aiming to restore movement and subsequently improve stimulation, nay be able to help improve development delays. While I do see this as well anecdotally in clinic, and there is a plausible mechanism, there is again limited data suggesting improvements long term.
      Long term, the helmet may help to improve quality of life, and the PT the developmental delays, so I think you have done the right things for your child given the information we currently have!
      Hope that helps,
      Christian

    • @AskDrChristian
      @AskDrChristian  ปีที่แล้ว +1

      Hi there Amboji,
      Two factors that influence skin colour that I'm aware of; sun exposure and genetics. I'm sure there are more, but these ones are straightforward.
      Genetics, can't help you there. If you and your partner are of a darker complexion, its more likley that bub is going to have a darker complexion. If you are both of a lighter complexion, then more likely to have a bub of lighter complexion. If you and your partner are of different complexions, then you get the skin lottery. My wife is from Taiwan and I'm caucasian, and we've ended up with two different skin type kids.
      Sun exposure, this one is actually important to ensure adequate vitamin D, but at the same time not so long that her skin burns. If you keep her out of the sun then her skin will not "tan", but then she may also miss out on adequate vitamin D.
      Now here comes a tricky part. I'm a little concerned that you are worried about a darker complexion?

      Firstly, sorry about my delayed response.
      As with any concerns, if you are worried about it, talk to your doctor. I can't see what is going on, nor feel what is happening. With cases like this, being face-to-face and actually seeing what is occurring is very helpful.
      It sounds like it could be a degree of plagiocephaly, but it would not hurt to have a review.

    Hi Doctor, my daughter is 18months old and I have noticed that the right side of her head is flat as compared to her left side. She’s born 6weeks early and I believe this could have been caused by how I breastfed her during the early days because I didn’t know any better.
    However, I have consulted two head doctors and they said it’s moderate but now helmets won’t make it any different but I feel like I have let my child down and troubles me every single day. Please do you think something could be done at all? Please answer I know it’s years ago you made this video but I had just come across it. Thank you

    • @AskDrChristian
      @AskDrChristian  3 หลายเดือนก่อน

      Hi there,
      Sorry about the delayed reply!
      Firstly, its very unlikely that it was caused by the way you fed her. Most parents feed in the position that is comfortable for their kids, and if she had some difficulty moving her neck, you would have adapted to that, not the other way around.
      At 18 months, you can still make a difference, but it will just be slower in recovery than if she were 3 months old.
      With moderate, they are correct in that helmets will make only small changes. I would consider taking her to a musculoskeletal therapist (chiro/osteo/physio - just make sure they have some training in looking after kids).
      Hope that helps. And one more thing: you have not let them down at all. You are still seeking out ways to help!

    Why is it harder to change the shape of a teens head than Someone who is below the age of 10? Since the head fuses completely after around 2 years of age so I don’t understand what difference there is.

    • @AskDrChristian
      @AskDrChristian  ปีที่แล้ว

      Hi there Yosief, and thanks for the great question.
      First up; the head does not fuse completely at all unless there is a serious medical condition occurring called synostosis. The bones of our skull are like plates that have join lines between them called sutures. These can move and assist with growth.
      If the skull was completely fused by 2, then our brains would not be able to grow after that as it would be contained within this fused skull. This is why it is a serious medical condition.
      And that's why we can have more effect when younger - the head is still growing. With growth, we can redirect and re-shape, but once that rate of growth slows down, so too does the rate of changes that occur.
      So thanks again for the question, and I hope this clears up some points!

      Hi Puneetha,
      This depends. After 6 months of age the rate of improvement slows quite a bit but you can still get some changes coming through. It really depends on how developed his head already is.

    @AskDrChristian my question is if the baby’s right side at 3 months is flat and we want to correct it by repositioning them to always face the left side…wont that transfer the flat side to the left side? Or do we alternate to correct the right side or just keep placing the baby on the left side to correct the right side flatness
    Really hoping this gets a response 🙏🏽

    • @AskDrChristian
      @AskDrChristian  3 หลายเดือนก่อน +1

      Hi there!
      We need to have them being able to face left to be able to round the head shape back out again. Ideally, they will be able to move to both sides, but we need to let the pendulum swing to the other side for a bit to balance the shape back out.
      The hard part is if you don't correct whatever is causing them to face that one side all the time, no matter how frequently you turn their head, they will just keep bringing it back to their preferred side!

      Hi there @Momeena7777 - very sorry about my incredibly delayed reply.
      How are things coming along?? This problem can go on its own, but there's a pretty good chance it won't...
      Personally, I'd get a review with a musculoskeletal therapist and get that range double checked. Babies at 3 months or so have a huge rotation capacity (back over their own shoulders - around 110 degrees!) and even a slight reduction in this may be enough to trigger a response.

      Alhumdulilah she is doing quite well! I'm from Pakistan and here unfortunately we don't have the facilities of helmet therapy but repositioning helped a lot. Flatness on the back still persists but my pediatrician made me understand that some head shapes are due to genetics too. There can be many factors to a head shape.Like other features of body nose eyes lips height it's not wise to expect to have all humans perfect round head shape and it has nothing to do with the functioning of the brain so just keep repositioning her head. Thank You so much for replying. I'm lil at ease as it's getting better but still not sure how it will turn out. She will be of 3 months soon. Can you suggest to me what I should do as we don't have helmets n other treatments of it available here. I would be grateful. Stay blessed

      Hi there!
      I'd get them assessed! While the rate of improvement may be slower, there may still be some change that can occur - especially if there is a musculoskeletal component going on behind the scenes!

      بكل صدق أريد أن أساعد ولكن أفضل شيء يمكنك فعله هو التحدث إلى شخص بالقرب منك حول ما يمكن القيام به.

      Hi there - and sorry but I have no idea! Over 90% of my patients are kids, so I tend to be up to date with kid stuff, not adults.

    I admit. It irks me when i see a baby with an obvious flat spot on their head. Not cool!

      Hi Andrew - it irks me when parents are told "its nothing to worry about" or that "they will just grow out of it". This is half-truth with risks of missing a window of opportunity to avoid that path!
