As an RN, this wouldn’t do this to the patient and this was the doctors fault and the doctor can lose their medical license for this. It was not the nurses fault.
All medical software I've worked with track changes that are made (usually crossed out if everything is deleted/discontinued or by being labeled edited). In the USA we aren't allowed to take pictures of patient records due to HIPAA, but if it had been printed out (with reason of course) that would be okay as long as it stays in the hospital. Just how it shows I edited this post.

I haven't watched this show so if let's say she's a nurse. After giving the medication she'll document: this Rn gave 2 mg of (medication name) to pt via IV line at x time per protocol.
Y'all, if you have problems with eachother and what to fight, do it in your own time and do it amongst you guys only. Don't put other people's life in danger.
As a nurse, it doesn't matter if you're just "following orders". We are the last end of defense for medication errors and we must verify if the dose is appropriate based on the patients weight/condition prior to administration.
In real life you can trace the change someone made in the computer though 🤷🏽♀️ But it's a good practice to have evidence for everything.. Just to be safe 😉
this happened to me. the doctor erased an entry which isn't allowed for obvious reasons. the hospital sided with her initially. thank god for my smartphone cause I knew the order was wrong.

Still, she could lose her license for negligence, because its the nurses job to understand everything about the drug they're giving, its interactions woth other drugs and the right dosage based on the patients presentation. You're not supposed to just follow the order given.
