I think the meaning is such that in pursuit of personal bliss (salma) you obviously end up leaving those who live in reality(his family) behind. They can never join your personal bliss. And if youre going to choose one or the other make sure you can live with the choice.
The movie is about drugs and drug addicts. The bliss is that Maybe its ok to get high all day and not take care of responsibilities… maybe being happy is what you choose …. Or at least thats what i gathered from watching it
