Why is that the other grandparents don't have white hair?
I don't care about spoilers so please someone tell me.
As far as we can tell, the Saitou couple had a good relationship with their entire family, so they are in good terms with them and why they have white hair (until Ine bumped her head into the hourglass and made her look as she was in the 1940's with amnesia). Satoshi was raised by his grandparents after his parents went through a nasty divorce, and so his grandparents raised him like he's their son and he saw them as his parents. Their hourglass probably made them a quarter younger of their real ages.
Not really I just realized and came up with theory that the apples are sentient....
Remember the episode where the Apple tree say thanks to Saitou's before thanosed. Okay another theory is that this apple came to a old couple that had a problem, for example:
Saitou's problem is actually the gramps since he had issue that he cannot make his wife/grandma happy since we all know Ine grandma is patient person due to gramps personality but the thing is how grandma behavior also make gramps felt guilt toward himself that he forgot many important event that could be done when they were young thus this apple help this couple but not extend their lifespan
Now Satoshi's on the other hand problem is more towards their grandson itself, you see Satoshi himself is older but he's not finishing his study yet and he also afraid that he cannot repay his elder who became the second parent for him therefore the apple give the couple to look like mom and dad look rather than grayish hair like Saitou's also Satoshi couple might be younger than Saitou's which I calculate their age not the same (around 70 while Saitou's is end 80s).
Or probably they were younger than main grandparents, so younger age

It's not really a good anime. It seems like a unique idea, but it never does anything....no comedy, no romance, no reflection of their lives, no action. People just don't even care that much of how they got young either. 4/10 anime
It's basically a Japanese PSA for people to get married and have kids. The author even admitted to this in his manga.
