Hi Lisa. You can do it. Tab your books, organize your notes in your book, and go to bed early and have a good sleep tonight. Remember 70% is an obtainable goal and even you have 45 incorrect answers, you still pass the exam so take it easy. Good luck with your exam!

@@nadinewalker9375 When you use 3M encoder, the software will take any keyword and then you have to go through a series of questions, we called it a logic tree in order to narrow down exactly the right code for the procedure. Let's say the patient has a laparoscopic hernia repair. Do not start with laparoscopic, always start with the terms that describe the procedure not an approach for example in this case" hernia repair" then it will prompt you to pick either open and laparoscopic then you pick laparoscopic. This way it will minimize the errors that you make while searching for code using 3M. I hope it helps.

Yes, in the real world, in the procedure report, the physician will certainly document his findings of the ejaculatory duct after duct radiography is performed and that is all you need for the supervision and interpretation. For the CPC exam, they could add just one sentence there is no evidence of a physical blockage and that is it. All I want to remind the student is to remember to read the note under the code.

Hi Rena. It is because the pathology report has confirmed it is the prostate cancer so you don't code the elevated PSA. Based on the ICD-10-CM coding guidelines, when you have a definitive diagnosis for example in this case a prostate cancer then you do not code abnormal test results, signs and symptoms. I hope it helps.

Hi Ameera, not every single question will take you entire 2 minutes; some questions for example relating to anatomy and physiology, CPC code symbols, rules and guidelines if you know only take you a matter of second to nail it. Also, my approach to tacking the case studies is the best and fastest approach that no one could offer an alternative. How long does it take you to highlight a CPT code? Please practice first and then kindly let me know how it goes a month later. I hope it helps. Practice makes perfect!

Hi. The elevated PSA is the main reason for the biopsy to rule out the prostate cancer, in other words, it would meet the medical necessity for the biopsy procedure. A benign neoplasm of prostate does not warrant the biopsy. If it turns out to be a prostate cancer then you would not report the elevated PSA because it's integral to the prostate cancer diagnosis. I hope it helps.
For case #3, how is the answer C and not D? The 52010 has duct radiography in it and 77440 speaks on radiological supervision & interpretation which is not mention in the case of being performed.
The physician will bill for the supervision and interpretation and the hospital still has to bill for the technical service portion 77440 for radiologic service as 52010 states that it is exclusive of radiologic service. The question did mention duct radiography was also perform to visualize ejaculatory duct system , in other words, the physician has to visualize the contrast image and report his finding because it is the main purpose of this procedure to inspect the ejaculatory duct system.
If you read CPT 52010 description carefully, it states that "... EXCLUSIVE of radiologic service" and it means that this code excludes or doesn't include the image 70, 000 series codes. I want you to open your CPT book, look the note under 52010 and you will see it instructs you to add 74440. I hope it helps.
When coding ICD-10-CM codes for the last case on this video, why do you choose C61 over R97.20? Are both not accurate? I'm confused why one is chosen over the other.
This is an ICD-10-CM coding guideline that you should know: Codes that describe symptoms and signs are acceptable for reporting when a related definitive diagnosis has NOT been established or CONFIRMED . In this case, when a malignant prostate cancer has been confirmed by the pathology report, then you do not report R97.20 (elevated PSA) which is a sign of the prostate cancer. I hope it helps.
Do we have enough time to write in notebook and write description and do coding like this way in main exam? Can we use highlighter notepad and pen and write each one and do like this way??
How long does it take you to highlight a keyword compared to the time you spend to read the entire operative report without knowing what your focus are? There will no faster way except my strategies, you can do your research on TH-cam? Many people who watched my videos already passed the CPT exam and left their comments under my video so you should ask them about bring highlight, notepad, ext. Good luck with your study.

In order to use 76942, the physician needs to document that " the image is a part of the permanent medical record and is reproducible should the need arise" because not every single US machine is able to produce the image document for medical record. If you don't have it documented then it is considered a part of the procedure. This is a documentation guidelines.

Hi Ameera, not every single question will take you entire 2 minutes; some questions for example relating to anatomy and physiology, CPC code symbols, rules and guidelines if you know only take you a matter of second to nail it. Also, my approach to tacking the case studies is the best and fastest approach that no one could offer an alternative. How long does it take you to highlight a CPT code? Please practice first and then kindly let me know how it goes a month later. I hope it helps. Practice makes perfect!

Hi Bala, the code 52010 describes duct radiography but excludes the radiologic service which is the 70,000 series CPT code. I hope it helps.

Hi. The elevated PSA is the main reason for the biopsy to rule out the prostate cancer, in other words, it would meet the medical necessity for the biopsy procedure. A benign neoplasm of prostate does not warrant the biopsy. If it turns out to be a prostate cancer then you would not report the elevated PSA because it's integral to the prostate cancer diagnosis. For case #3, how is the answer C and not D? The 52010 has duct radiography in it and 77440 speaks on radiological supervision & interpretation which is not mention in the case of being performed.
The physician will bill for the supervision and interpretation and the hospital still has to bill for the technical service portion 77440 for radiologic service as 52010 states that it is exclusive of radiologic service. The question did mention duct radiography was also perform to visualize ejaculatory duct system , in other words, the physician has to visualize the contrast image and report his finding because it is the main purpose of this procedure to inspect the ejaculatory duct system.

Case 3. 52010 or duct radiography is mentioned. In question also they mentioned duct radiography was performed. Then why did we add 74440 sir....
If you read CPT 52010 description carefully, it states that "... EXCLUSIVE of radiologic service" and it means that this code excludes or doesn't include the image 70, 000 series codes. I want you to open your CPT book, look the note under 52010 and you will see it instructs you to add 74440. I hope it helps.
When coding ICD-10-CM codes for the last case on this video, why do you choose C61 over R97.20? Are both not accurate? I'm confused why one is chosen over the other.
This is an ICD-10-CM coding guideline that you should know: Codes that describe symptoms and signs are acceptable for reporting when a related definitive diagnosis has NOT been established or CONFIRMED . In this case, when a malignant prostate cancer has been confirmed by the pathology report, then you do not report R97.20 (elevated PSA) which is a sign of the prostate cancer. I hope it helps.

How long does it take you to highlight a keyword compared to the time you spend to read the entire operative report without knowing what your focus are? There will no faster way except my strategies, you can do your research on TH-cam? Many people who watched my videos already passed the CPT exam and left their comments under my video so you should ask them about bring highlight, notepad, ext. Good luck with your study.

For case #5 why is the ultrasound guidance code not 76942?
In order to use 76942, the physician needs to document that " the image is a part of the permanent medical record and is reproducible should the need arise" because not every single US machine is able to produce the image document for medical record. If you don't have it documented then it is considered a part of the procedure. This is a documentation guidelines.
