    I was in tears watching this - thank you for being so clear and thoughtful in your explanation. This is EXACTLY what I’m going through and I can share this with anyone who needs to know. I’m hoping the exercises and movements will help and hoping I can find some helpful treatment. THANK YOU again for this video. I need some hope to cling to and this helped.

    I was finally diagnosed with this condition late last year. I think I've had it for a long time, but the symptoms have come and gone over the years. Of course I went through multiple courses of treatment for things like bacterial infections first, but I knew it wasn't any of that due to the symptoms and finally a urogynecologist suggested it was a neurologic issue. I started looking around myself and found pudendal neuralgia. It matches my symptoms almost perfectly. A burning pain - for me in the vagina in the urethra area and closer to the perineum with urinary urgency - all of which is made worse when sitting and eases or goes away when standing, lying down or sitting on the toilet. The pain is usually completely gone overnight and slowly comes back as I start my day. Basically everything he said.
    I went to a specialist for multiple nerve block treatments, I use muscle relaxing suppositories at night, take pregabalin and I'm doing pelvic floor PT - which has revealed a lot of tension in my pelvic floor. My therapist thinks all of the coughing and sneezing that I did when really sick made the pelvic floor very tense which irritated the nerve.
    I will say that all of these things have made an improvement. I'm still not pain free, but it's much better than it was before I started treatment so hopefully with time it will ease and I can feel normal again.

      I am facing the same, I also have PCOS So I thought it's pain in my ovaries but, I feel fatigue and pain around my vaginal area and in my hip, I've been listening to anti cancer frequency thought it's cancer, but I get relief when I listen to peripheral neuropathy frequency by Good vibes on TH-cam. But I don't listen to consistently that's why the pain comes back.

      I just want to know why us as the patient have to diagnose ourselves then find a dr That one even knows about the pudenal nerve and that who believes us?!! You would not believe how many specialists I've had to go to and still have no diagnosis. Or actually somebody that even knows what the diagnosis is or what It means much less how to help me!

      @@lindsaymarie7238 Hi Lindsay, I'm so sorry to hear this. I know exactly what you mean. This is the 2nd urologist I'm seeing. The only thing he did was give me antibiotics. He claims my prostate is inflamed, but when I said I want a ultrasound of my prostate he said no it's not necessary. He could tell with a rectal exam. Know matter how many times I go over the symptoms with him. It's like talking to a wood chuck and a coal stove. I'm in pa. My neurologist gave me a specialist in Philadelphia but my insurance won't cover it. What else have you gone through?

      My insurance company demanded an EMG before I could have a block. I went to the EMG specialist, and I had to show the dam doctor a picture of the nerve. He never heard from it. I was beyond pissed off. I live daily in extreme pain, waiting to have this block done.

    I am a 70 years old retired woman surgeon. I have had irritable bladder and detrosal muscle symptoms for a few years. About two months ago I started experiencing perineal pain and severe urethral irritation with frequency. I had no urinary/bladder infection or cystitis. I saw a urologist and a gynaecologist who found all is normal including abdominal and intra-vaginal ultrasound. Paracetamol eased the pain a little. I researched my symptoms and believe I had pudendal neuralgia which as you say is twice as common in women but frequently passes undiagnosed. I started exercising my pelvic muscles and continued with my stretching exercise routines. The pain subsided completely within a week and I have been pain free since then. Of course it may return. Thank you for shining a light on this distressing condition that is rarely considered particularly by general practitioners.

      Nahid toubia , can u please share Ur exercises. I'm a physician and started having it 4 months back . I have endometrios , deep infiltrative and had a UTI after ocps withdrawal after which I strated having severe right labial and right sides vaginal pain which would get worse on sitting , improve on walking. Also had burning at the same site. Later started having pain in medial calf and last two toes , the sacral plexus has something to do with it
      I consulted pain management and they said it's pedundal neuralgia , took amitryptalline and walked , things got better but now again after I sat for a long time things got worse.
      Would be a great help if u can let us know how u got rid of it

    Thank you for your videos. It is very informative 👍. I fall over my back 2 years ago and after three weeks of the accident I felt pain around my vulva and during urination and around urethra. My doctor told me I have descending perineal syndrom and rectocele and the descending caus a nerve stretching. And told me that the fall was only a trigger point for the prolapse to become systematic. My question is ? Is it the fall or the prolapse which caused this firing pain??? And does physics therapy can help with the enormous pain I have lived with the past two years.

    I’m in a FB support group for PN and in all of us there are so many differences in what aggravates it, what helps the pain (including meds) which areas are painful and which are pain free. However, in 100% of cases, upon waking the pain is entirely absent, for moments only. What happens in the brain during sleep (not relaxation since we’re still relaxed when the pain reappears just minutes after waking)? If we could answer this, that would be valuable intel on what is going on that so affects the nerves to cause such horrific pain.

    4 years of high level daily pain, they say I'm full of trigger points. Where do I even begin? The pelvic floor PTs here just don't have the experience, and I've felt like I was on my own trying to fix this. I just started seeing a sports rehab PT and he's been so much more proactive. First session he did trigger point work with needling ecurrent on my glutes and lower back. This week we're doing the obturator internus. I wish I hadn't just settled for what insurance covered and had just payed the money to see this sports rehab PT from the beginning. Way more experience and sometimes you get what you pay for I guess. My pain began after running marathons without ever stretching or doing any kind of strengthening. I hope someone reads this and learns from my mistakes. Pudendal neuralgia is no joke

    I developed this at age 16 after suffering chronic constipation for a long time and running excessively, stress plus a strong enema at the hospital to try and relieve acute constipation. Nobody could tell me what caused the condition. It made my life hell. I had to drop out of sixth form and give up on going to university. I found it so stressful that it lead to chronic fatigue syndrome.
    I now understand this condition and started to in early 20s thanks to the headache in the pelvis book.

      I think meditation with the pain is very important to be able to let go of the resistance to it and the guarding against the pain which keeps everything tense down there. Paradoxical relaxation you'd call it. For me stretches tend to aggravate things so I just do gentle yoga in the mornings.
      The meditation that I find most beneficial is called soften and flow. Focus on the area of pain, breathe into it, then on the out breath gently say in your mind soften and flow. Don't expect miracles but over time as you get more used to sitting with the pain rather than distracting or running away from it, things will improve.

    After having a bartholin gland abcess for 3 weeks, two incisions and drainage that were unsuccessful and then it drained on its own a week later. 3 weeks after this I started experiencing all the symptoms that are explained in this nerve irritation. My gynecologist pressed on the nerve but no pain, but it comes and goes and is less in the morning and gets worse throughout the day. This is a tricky situation when nothing is felt at the gynecologist in the morning but is painful for the rest of the day.

    Look up the sacrotuberous ligament. Chiropractor did a technique to release that ligament and it instantly gave me relief from the sacrum and groin pain from the pudendal nerve.

    Thank you. Mine isn't worse when sitting. It's worse when standing or laying on my back. Laying on my tummy feels best. It's been 2.5yrs and it started after rotator cuff surgery. I'm getting a diagnostic pudendal nerve block next week. I'm in perfect health other than this. I'm praying 🙏

    Pelvic floor therapy has helped me reduce most symptoms by 75%. I went 4 months with severe pain.All test came back showing good health from 3 different urologist. My 4th urologist suggested pain management and pelvic floor therapy. Starting my third month of treatment now. I have good weeks and bad weeks but no where near the pain before treatment started.

      @@hcs.c 95% better now. Little to no pain each day. Pelvic floor therapy in addition to breathing exercises to calm my nervous system really helped. I used wim hof method for breathing and a program called heal pelvic pain for men (online course) in combination with a local physical therapist. I had three flare during the couple months of recovery but now they have stopped.

    I think this is what I’m experiencing. After two years of pain and research I feel that this moist describes what’s happening to me. I hope I can see a doctor soon and mention this. I’ve actually lost my physically active job and health insurance due to these complications.

    I'm a 26 year old man and not ashamed to admit but I've felt this condition on my right pelvic region, feeling a sharp, prickly pain or sensation emanating above and intermittently towards the right testes. Thank God the "ball" itself is fine but still the nerve pain persists, at least occasionally now thanks to more stretching.
    Any other recommendations would be helpful 🙂

    A can relate to the guy you mentioned at the end of the video 😔. Same symptoms. I told the Doctor this could be my problem but he doesn’t now much about it.
    Doctor keeps telling me all my exams came up good, but I know what I feel and it gets me depressed having this pain and nobody be able to help.
    How did the guy you mentioned heal of this terrible problem?. I need help 😔.

    So grateful you explained this! Mine is constant, sharp. Worse than childbirth on BOTH sides.
    Everyone says they have pudendal neuralgia like I do but they’re symptoms are very mild and only occur during sitting.
    Mine are constant and torturous

    suffering from bilateral pudendal neuralgia right now as well as sacral neuralgia, waiting for infiltration first, but my doctors told me I need surgery and I am devastated. Its gonna be my 4th major surgery [not for pn but in total]

    Hello doctor, I fell of from stairs and got myself injured and sit bone near tailbone got injured, but after few years i started getting bladder spasms with frequent urination and stiffness near tailbone. It happens mostly with sitting and during night time i feel completely fine.       I just learned my systems were from the sacrotuberous ligament. Chiropractor did a technique that released the ligament and gave me instant relief. The ligament is attached to your sacrum so your sacrum could be moved just a bit.

    Have recently been diagnosed with this . I have s1 and s4 disc bulges , and the nerve has been affected somehow . I get the feeling in the end of the penis , and it seems to be worse in the morning . When I walk and move gets less . I have been diagnosed with prudential neuralgia; but still having issues helping it .

      I've noticed some relief with this medication as well. I'm on a very low dose, but I started feeling improvement within a week. And far less side effects than I had when I was on gabapentin.

      My dosage is only 50mg per day and thankfully I haven't had any weight gain or increased hunger. However, I also take hydroxychloroquine daily as well and that suppresses my hunger so that might be contributing.
      My doctor told me it could take a month or more to get any relief from the pregabalin and also said the dosage might have to be increased. So hopefully with a little more time it will start to help you.

    So you guys can help if the compression is muscle related, but what if it's compressed in Alcock's Canal or between the sacrotuberous and sacrospinous ligaments? In that case wouldn't it make most sense to get an ultrasound guided prp hydrodissection?

    I have similar extremely bad pain in my genital and started in year 2020 when i was sick may be due to corona and then i started cycling too much in weakness and everyday i come home with swelling in genital and soon i started having pain after sitting in morning in office for few hour and initally i thought its urology problem and was given too many strong antibiotics for year , then after 2 year of very bad pain everyday i realize myself that in night pain goes away on sleeping and its not antibiotic which is helping and no neurologist or urologist able to diagnose every report come good and doctor say i am perfect , this is my exact problem for 3 years and i am still suffering every day , please help me with any medicine or exercise as i am very depressed

    Pudental nerve irritation can cause recurrent UTIs in both men & women. Urologists often diagnose this condition as Chronic pelvic pain syndrome (CPPS) in men. I have been suffering from cpps from last 13 years. I am 32 currently. Pain used to happen in my groin area & it was in vericose vein. Doctor classified it as vericocele but even he wasn't sure about pain because it was constant. Thankfully, I had the surgery in 2014 & pain never occurred again. I still have recurrent UTIs though. I would have a pelvic MRI & will look into this information that you have shared. Urologists across the world are stupid it seems.

    This is an incredible light bulb moment for me. I, a man, was sitting with my right leg crossed underneath my left thigh, for years.
    That must've contributed to what I went through recently. Am improving, just some pain and discomfort which will disappear, I believe, with simple correct sitting.
    Sitting on a travel pillow (donut u shaped) is recommended. Thank you doctor.

      @@SouravMax-dt8xn healing very well thanks. I think what happened was stress, caused temporary issue for months with peripheral nerve, which affects pelvic floor function, and continence.
      As body went into repair mode, everything got better. A big thing to do, at least a few times, is pelvic internal therapy.
      Use finger or pelvic wand, even better, into anus, and do myofascial release of any tight pelvic muscles.

      @@SouravMax-dt8xn I had the incontinence, pelvic pain, couldn't walk properly for a couple of months.
      I'm not an expert, but it sounds Ike it's your pudendal nerve. Fortunately, the pudendal nerve regrows and heals.
      The amount of time it takes differs depending on severity, but you will heal. For now, meds can help. I took pregabalin, was good, still taking for a bit longer.
      You'll get better for sure.

    I get this sporadically, I’ll feel it for a few seconds, then I won’t get it again for months, for me it usually happens when I sit down too long and then I stand up and stretch my abdomen, it’s like there’s a rope attached to the left side of my lower left pelvis and runs down to the tip of my penis and it’s getting pulled casing a burning stinging pain on my penis tip to my lower left pelvis.     I don't have pain. But get relapses of back pain and have chronic hip pain. When I get relapses of back pain I get a complete erectile dysfunction. The function comes back somewhat when the pain subsides. But I have to tilt my hip/pelvis sideways while trying to achieve an erection. If I don't do this, nothing happens. I also sometines get like static electrical impulses pulsating down into the left leg and/or the penis. Pretty subtile but during these periods I also get a complete ED. Since I at these periods doesn't get nightly erections either it has started to shrink in size when I have periods when I can get it up. I think I've lost 2 cm in a year. >.

      What are you doing to control pain. I too am suffering since 14 years. I Tramadol both the times, siltz bath as and when required. No sitting at all. Hollow cushion for limited sitting for dining etc. Pain is unbearable during cold months and in hot summer I am able to manage better. Are you in India?

      I am not in India. I rely a lot on my guashua, simple stretches, yoga, prayer. Ice packs and warm baths (Epsom Salts or Soakness). I receive Pudendal blocks at the hospital when I can no longer tolerate the pain. I also get therapeutic massages which help the myofacial restriction.

    Why Is nobody talking about the fact that sexual activity is a definite no go and non-existent and also having bowel movements are nearly impossible now?!?!     I’m 32 y.o man who have been suffering from this condition since 5 years. I went to many doctors and they were thinking I have a psychological problems and there’s nothing with me until a good urologist told me it’s pudendal neuralgia 😢 what really make me sad that there’s no cause for my case I’m not a bicyclist, I had no surgery or accident. It just happened to me for no reason 😭 why! Why! 😩 will it become worse by the next years?! Nobody even my family believe me and think it’s psychological while I’m suffering everyday. I haven’t been in sexual relationships because of it and I’m afraid to get married. Anyone can tell me if it hardly affects sexually?

      I’m 33 and I’ve been dealing with this since 17. It just happened and no doctors could ever help. Sex can hurt sometimes but not always. It can be irritating after sex a bit. I still need to go and get diagnosed with this because I believe this is what I’m going through. Hope your situation gets better. This really sucks

    Pudendal Neuralgia is TMS (Mind+Body Syndrome). AidPlan does not recognize this, and the effectiveness of the treatment is thereby limited.

      @americandream9473 Anxiety is usually a part of the TMS mechanism, yes. I had pudendal neuralgia on my left side for years, diagnosed by numerous professionals. It was all TMS, though, and disappeared completely after 2 months of following a TMS protocol.
