I am making this short series on infant formula feeding, not because I endorse formula feeding, but because I recognize a small proportion of mothers need to use some infant formula despite their best efforts, and they need support as well. It is very important to be meticulous about sterilisation of the bottle etc (which I will outline in a later video), and it is very important to maintain breastfeeding as best as you can, even if you do need some formula feeds (and please be careful when you make that decision, as giving more formula will drop your milk output). The family members have to support the mother's efforts to breastfeed, and avoid pushing them to give infant formula.
In this video, I discuss how to decide how much to feed the baby. Please individualize to your baby's needs, and follow the feed pattern similar to what I have discussed earlier (regarding feed volume and gap between feeds) to avoid colic, reflux etc. I hope this series is useful, do share.
It is very important to get the cues right and not to push the baby to complete the feed volume. Over feeding can happen even with breastfeeding (I will share the video link below). It is very important to educate the parents right from the start regarding the feeding pattern, so we can prevent reflux happening. In the first 7-10 days, we expect the baby to feed almost every 2 hours-this is because the milk production is increasing, the baby has a small stomach and the volume is gradually increasing. This frequent feeding pattern helps to empty the breasts and encourages establishment of lactation. After the milk output has increased (we can make this out by the improving urine output, satiety, sleep pattern as well as the weight gain during clinic reviews), the mother can be advised to relax about the cue interpretation. Most experienced mothers start spacing feeds reasonably well at this time, and breastfed babies can go 2-5 hours between feeds. Educate the mother that the longer the gap between feeds (with upper limit of 5-6 hours), the better it is in terms of colic, reflux and stool frequency/nappy rash. Unless we try to space, babies keep persisting with the pattern they are used to, and since the milk output has increased, they get excessive milk at a high frequency. This impacts on their ability to digest the lactose load as well (as this is gradually increasing in the babies, and a rapid load will mean they exceed their ability to digest the lactose)-this leads to the physiologic lactose intolerance that we referred to earlier (will link the write-up on lactose intolerance). The babies have an active gastrocolic reflex, and the more often they feed, the more often they pass stools. Combining this with the physiologic lactose intolerance, you have the higher risk of diaper or nappy rash as well. Colic tends to be more related to this and because they feed before the stomach is emptied, reflux occurs as well. As you can see, these patterns are all linked. And it is important to start the education early, so the mother watches for signs of improving output and avoids entering this cycle.

You could try giving 60 ml every 3-4 hours and monitor weight gain (current) after 1 week. Decide based on this gain, as comparing from birth weight at this stage is difficult. Don’t force feed, take precautions to reduce reflux (review my videos on reflux)

Hello sir.. my baby is 7week old and he was in mix feeding from birth due to his stay in NICU for Jaundice.. Yes, you can. But try to pump so you increase your milk supply and use less formula

Hello sir, I have 2 month 10 days twin babies both are on formula feed only. Twin 1 was in NICU for 1.5 month whose birth weight was 935grams. Now his weight is 2.300 kg. He consumes 45ml of Nan Pro 1 for every 2 hours. Twin 2's birth weight was 2.35kg.current weight is 3.7 kg. she is consuming 75 ml of nan pro 1 formula milk for ever 2 hours. Even this continues during night.

Crying more at this stage is often expected due to colic. You could consider a little expressed breast milk if essential to top up but avoid formula in this situation, your milk will drop off rapidly

It can be feed aversion related to previous reflux. Yes, you can introduce solids and also consider water by sipper cup and then expressed milk by cup

What do you mean by not gaining weight? It depends on your expectations. Give breastfeeding and 60-90 ml Expressed milk 3-4 hourly, review weight gain over 2 weeks (express every 3-4 hours). Avoid formula

Hello Sir, My baby girl is 2 months old and drinks only 60 to 50Ml of formula milk in 4-5 hours. Her urine output more than 7 times a day. On 13th Oct we weighed her which was 3.740kg. I am worried about her growth. Her suggest.
Growth depends on many factors. Your baby is growing well, and is probably starting to refuse feeds as a result of earlier overfeeding. Allow a longer gap, let baby start regulating intake (don’t focus on volume, keep the gap even if a smaller volume taken unless baby demands). It will normalize over a few weeks. Hello sir, my baby had a poor weight gain so the doctor suggested formula feeding.. we made a mistake of introducing bottle feeding to the baby who was only 3 weeks.. now at 6th week, they say that the baby might have nipple confusion and is not feeding from my breast (they didn’t check the latching or how she feeds).. we have increased the formula feed and I’m expressing my milk twice a day to feed her.. (I get 60 to 110 ml based on how often I feed her)..at early hunger signs when I breastfeed her she latches and drinks but starts crying in 10 to 15 minutes so I give her formula in a bottle..but when she is completely hungry and crying she does latch and drink milk and sleeps for 2 to 3 hours..She still has not reached her ideal weight… she also has acid reflux and spits up after every feed…Now I’m scared to breastfeed her as she may loose more weight..How can I overcome this situation and exclusively breastfeed her and ensure she gains proper weight.. How do I know for sure that she has nipple confusion.. will meeting a lactation consultant help?? (to check the latch) Please, I request for your guidance..

Babies tend to be restless at this stage due to colic, and many mothers interpret this as hunger. Review the weight gain while you were giving only breastfeeding and if satisfactory as per centile, avoid formula. You can express by pump and topup your milk instead of formula for next few days as baby might be used to this (and you need to improve your output again as it would have dropped when you introduced formula)

My baby is 23days old & fully dependent on formula milk. Initially, She took 30ml in 2 hr. Since last 10days, I am giving 40ml every two hour. However since last 2-3 days, specially on night.. You need to consider using a pacifier and wait longer between feeds (3-5 hours between feeds). Short frequent feeds is not ideal and baby can’t take more this way. Delay first and then increase

The growth is good, intake is reasonable, hope you are mixing powder as per instructions. Try to increase the volume when the gap is more to 4 ounces but don’t force feed. Take measures to reduce reflux, which can reduce baby desire to feed

And since I started biogaia probiotics he has started spitting more. Also he is coughing forcefully during and after feeds though sometimes. I give him 90-100 ml every 2 to 2.5 hours. Also he feeds less at night. How to go with reflux and what can help with the cough? He also has mucus at times.

Should not be a problem as you are related and know the risk of infection etc, you can review her bloods for Hepatitis B, HIV etc during pregnancy but that was a while ago. It’s your choice ultimately as there is a risk from these if unknown

You could consider introducing weaning foods like pureed vegetables after 4 complete months (17 weeks), and also try sipper cup. So her total milk take per day is between 500 and 600 ml
Babies go through phases and she is learning to regulate better. Weight gain is good but possibly more than the trajectory, so allow baby to regulate, don’t force feed, allow spacing and don’t be too focused on volume as long as weight pattern is ok (review this below)

It is a similar volume given at a more frequent schedule maybe due to colic. Try to wait longer as possible and establish the 3-5 hour pattern, use pacifier to try and delay feeds

Hi, in some mothers, it takes a bit longer to establish feeds, you need to ensure adequate latch, with supervision if needed, and use breast pump to empty after each breastfeed, at least 3-4 hourly. If you are getting 10-15 ml each time after expressing to empty 2-3 times, it is likely to increase rapidly and no need for formula (check weight loss of baby, monitor urine output every 4-6 hours as indication of output). You could consider formula as a bridge till this succeeds (next 3-4 days) if worried, don’t give more than 60 ml per feed after trying above, and stop soon after your output improves (if your aim is exclusive breastfeeding)

If you give 3-4 good solid baby food mixed with milk and yoghurt, you don’t need much milk. Monitor growth and don’t force the milk but try adding partly while cooking

Weight gain is quite good, better than what we see for most small babies-was he IUGR or preterm? Did you try expressing regularly to increase your output? I would suggest doing that and not to increase formula any more, to avoid overfeed

My baby is 22 days old now.. my breast milk is too low.
So how much formula milk will be sufficient for my baby... Right now I m giving him 30 ml (3-4 times) a day along with breast feed in between.
But I assume his tummy is not full as he does have a sound sleep in the day time and in night he usually doesn't sleep. Kindly identify the reason for the same.

Try using expressed breast milk instead of formula so the taste is familiar. Sipper cup is an option. You could consider early weaning foods after 17 weeks of age, and manage with expressed milk and solids
Sir should i try to change the brand of the formula feed. I was trying to give similac advance 1.whether it would be proper to introduce solid before 6 months. Kindly suggest what kinds of solid food i can feed her with?

Hello doctor. My baby’s birthweight was 3 kg. Now she’s going to complete 10 months . For last 3 months her weight is not increasing. It’s 7.4 . She’s not taking spoon feeds but same if I put in bottle she drinks. She takes 80 ml formula milk for each feed (3 times a day ). I’m worried. Please guide . Thank you.
Giving solids in bottle is bad for teeth. Wait more between feeds, be patient, focus on improving eating pattern first and then watch the weight. Increase to 150 ml 3-4 times formula-are you still breastfeeding?

If you mean hardly any breast milk, try expressing with a pump to stimulate and start formula. Ragi and other foods close to 6 months is better

He is not takinh enoigh formula.was never on breast feeding.
He takes 100 to 150 ml milk at the interval of hours. Is it normal.he os 8.9kg and his birth weigjt was 3.2 kg
It appears he is overfeeding as weight gain is excessive if 8.9 kg at 4.5 months

My baby is 51 days old. She consume 60 to 90 ml milk in 2 to 3 and half hours gap. Current weight is 4.2 and birth weight is 3.1. But she had weight loss in first month due to infant jaundice reduced weight @ the time of discharge is 2.6 kg. She is refusing consume more milk. Kindly help us. Is there any issue with current weight? Should we need to force to consuming more milk?
The weight gain is good, try waiting more between feeds if baby no keen, then might complete 90 per feed consistently. Are you breastfeeding as well?

In addition to formula milk 2- 3 times breastfeeding also but milk quantity is very low. And my confusion is how much is the ideal quantity of formula milk need to feed. Most of the time baby goes of to sleep while feeding. So i am very much worried about babies health and weight gain.
