I did only watch like 3 episodes, so I cannot really say much about the entire thing. however, this is kinda sad in its own context when it comes to mami(what happens to her in the show is the one thing I know btw) and madoka. basicly, madoka sees her fall and from her just starting to roll, it is heavily implied she jumped down herself. but when they meet, it is already too late. and then she is in this mess of people fading out of existance and vanishes herself at the end. more or less never jump into hell to pull someone out, it does not work
