    Hello, I really like your video. Please, what documents do they give at the hospital, like forms they give in the hospital to be filled?

      Hey like o said in the video ,when I was 7 months pregnant that’s when I went to fill the forms as my doctor requested.When going to give birth I just went with my mutterpass and insurance card and baby clothes
