Oh there are PLENTY of examples of deaths (and soft-locking the game) in older Lucasarts games. SMI aside, Maniac Mnasion is a prime example. You can also lose in just about all Indiana Jones games, Zak McKraken, and even Full Throttle (though you immediately respawn and get to try again).

Not as fun as the Sierra adventures, but at least they're less frustrating.
