6+ years out and am biking 80 miles per week. Three months after a very successful hip replacement I went down on the ice in our driveway and fractured the femur on the leg with the hip replacement. What a nightmare. My advice is 1) If you’re overweight, lose weight. 2) Treat your PT like it’s a full time job I’m now 69 and working towards a 70 mile ride to celebrate my 70th birthday Best of health to all
I'm 12 months post-Anterior Total Hip Replacement, and I can say the results far exceeded my expectations. Besides having a world-class surgeon, and a post-surgery physical therapist that was next-to-none, they both gave me the best advice I could have ever received. First, about an hour after waking up from the surgery, my surgeon had me out of bed walking up and down the hospital corridors with the aid of a walker. It was out-patient surgery, so several hours later I was on my way home. My surgeon was adamant that approx. 4-5 weeks PRIOR to surgery, that I go through a series of exercises to strengthen my glutes, hamstrings, quads, etc, and it helped 100%. He said the stronger you are before the surgery in those areas, the easier post-surgery will be. The PT also put me through a series of exercises including those mentioned by Will in this video, and I did them twice a day, religiously. In 3 days I was able to put the walker away, and only used a cane. 9 days after that I was walking normal without any aids at all. 9 weeks to the day after my surgery, I was back on the tennis court playing doubles, gingerly, of course, but gradually I got all my leg and hip strength back. Follow Mr. Harlow's advice and do these exercises prior to and after the surgery. You'll be so glad you did.
Thank you for sharing your experience, I'm so pleased the surgery was such a success for you and that the results have been so beneficial - well done on all your efforts, it just shows that hard work pays off :)
I don't know if that would happen if you had Complete Knee surgery . Knee surgery is one of the hardest, painful , surgery out there . I believe hip surgery "alittle" easier and it all depends on how old you are . There's a lot of things factor into all these surgeries. In the near future I might need a Hip surgery and I know for a fact , I will need 2 complete Knee surgeries . And you have to remember Everyone situation with pain , and anything else , aren't all the same . This is what I had read , spoke to people who had hip and knee surgeries. Everyday for several years, I have been doing alot of reading , asking questions , spoke to Physical Therapist about all this . I even spoke to , two Orthopedic surgeon. I am NOT disagreeing with you, I am only saying , what I had done for research, etcccc.
As a fellow physical therapist who treats many patients right after total knee and hip replacement surgery I can tell you that the glutes are one of the most undertrained muscle groups. I also use the clam exercise as a starting point for my patients and then eventually transition them to a standing hip abduction to get them comfortable with starting to put weight on their leg again! I always remind people to train BOTH sides to keep their hip stability and strength balanced. Thanks again for this great information Will! Keep up the great work in helping our patients!
I had full anterior Hip replacement 2 1/2 years ago which was very successful. I was not referred to physio at all & I have started having pain as described. Thanks for the video
Hello! I have been trying your exercises. I am 70 and have had a total hip replacement. Your book arrives tomorrow!!! Yay! I think you are going to really help me!! Thank you!
Great exercises!! Thanks so much! I'm 59 yrs old & have AxSpa, (An autoimmune disease,) and because the disease breaks down bone, I had my 1st hip replacement at age almost age 50. 2nd one was at age 56. These exercises were given to me to do after the surgery the 1st time but not the 2nd time. So i'm happy that the 1st P.A. showed me them. I'm glad u are showing these exercises because not all P.A.'s tell u about these and should!!
I'm pleased they were given to you for your 1st operation, so that you had the information for your 2nd rehabilitation. I agree, not everyone is provided with the same useful information, so hopefully my video can reach those that need the additional support!
I hope they prove beneficial for you - I also have the following videos on knock knees and knee arthritis which may be helpful! th-cam.com/video/MUm256_4dek/w-d-xo.html th-cam.com/video/-PTzzM31O7c/w-d-xo.html th-cam.com/video/SdH4RDjvBgs/w-d-xo.html
I'm in my 30s never had hip replacement surgery but I am healing from sciatica and a herniated disc and this stretch doesn't seem much but after the first 5 days of doing this stretch 2 times a day I could definitely tell there's was a huge improvement I feel better already thank you
4 years post hip replacement, and no one ever offered these exercises for me! Still getting at home PT 2x week and for sure will add these! Thank you, Will...you seem to always be "right on time" for me! ❤
I had both hips replaced and it was never suggested that I do an exercises to strengthen my glutes. In fact, at a follow up after my first hip replacement the surgeon told me my glute muscles were weak but offered no advice. Furthermore, neither of the 2 surgeons explained why we don't have the same range of movement after the operation or that it is permanent. If you check on the internet, all that is available is information for the first few months post surgery. I would love an answer to question of reduced range of movement.
I had a replacement hip 16 years ago and it was at that time that physio was given afterwards. These exercises except the resistance band one were what l was advised to do. It's brought back memories. Tough at first but an absolute must to do after hip replacement. I really benefited from them. Thanks for the memories Will.
Thank you Will for such helpful advice. I was given all those exercises but no one mentioned what I should or shouldn't feel if I was doing them wrong.
This video has answered issues i have had last 18 months. All 3 parts gluts, leg and back. I had hip replacement after avasular necrosis in hip. Had 25% muscle in hip region at time of surgery November 2021. Thank you for exc
Thanks Will One of my neighbours is having hip replacement surgery this coming Thursday but I feel its too soon because she had a knee replacement just over a year ago So the hip replacement is on the same leg
Yes, as a rural dweller, post-op rehab is difficult or non-existent. The original (bad) hip was a life long internally rotated joint and i still (9 months later) struggle with active external rotation. I also have L5 bony metasteses that make clams uncomfortable, better if i deliberately tilt my pekvis forward. Ill have another go at the exercises with better analgesia
One day I'll end up with a Right Hip replacement :( . And 2 knee replacement :( *Dealing with Trendelenburg Gait* . *I have weakness in the Gluts 100% , especially my Right Hip* *I haven't had Any Surgeries yet* *I am doing the Clam, each and everyday* I need a miracle and a lot of prayers!
I'm 14 months out from my replacement. Mine was not elective. I fractured my R femoral head after taking a fall while standing around on quad skates. I had just completed a two hour skating session, relatively incident free. I also keep up with ballet barre exercises, and get compliments on the shape of my glutes despite my age. In rehab I was able to take stairs the first time the P.T. asked me to attempt them.
My PT showed me these exercises 15 years ago after my first (left) hip replacement. I'm not able to do the clamshell yet (following my right hip replacement three weeks ago), because i have a six week restriction on abduction exercises as there was some bone fracturing during surgery. But I will be doing these after my restrictions are over. I can do the bridge and buttocks squeezes until then.
You're so right! I couldn't believe that I couldn't do a simple clam after surgery! Now what do I do about the trochanteric bursitis that's persisted after replacement?
I've had a hip fracture and a dynamic hip screw replacement, so I'm on hip restrictions for 6 - 8 weeks. I imagine I can do this 12 weeks post surgery.
I've never had Bursitis before but I developed some bad upper leg Bursitis about 3 months after my 2nd hip replacement. This has been almost as painful as pre-replacement and certainly has restricted my mobility. My doctor advised me to stop doing any hip replacement exercises until the bursitis settles down so as not to cause further inflamation. I was given a cortisone shot and while thi helped, it did not seem to last very long. After 4 months I'm about to get my 3rd shot so hopefully this will bring the inflamation down to where I can start using your excersise tips. In the meantime, I do see what you've mentioned regarding extra pain in both my knee and lower back. Thanks for you tips and I hope I can use these soon. Cheers.
Great! Builds on what I did with the NHS physio. Just a question - after six months I still have fluid around my ankle. Any exercises I can do to help solve this? Elevation alone doesn't seem to help much. Many thanks.
Thanks for these Will. I was not taught that second exercise and still have to work constantly on maintaining glute strength after having a fractured neck of femur pinned in 2012. I’m sure it will help me.
Thank you sgain. Sacrum fracture 12 months after THR. Yes strong glutes are essential, i intuitively felt it but thank you for this great reminder and thus protecting us.
Brilliant Will, was given these exercises, plus resistance band for clam and bridge, and told to push navel into mat to work the core doing the bridge as well as buttocks squeeze which was helpful, also physio noticed my operated leg remained weaker so doing more reps with the clam on that side and extra pulling on band tied around the leg of a table ... it's taken frankly a really long time at my age plus additional injuries still things are progressing (also sit to stand excellent but that's not glutes I know.) Love your 3 tip Friday e-mails!
I'm pleased my video aligned with what you were told previously, well done on the efforts you've made - and I'm very pleased you've found my e-mails helpful, that's great to hear!

I have had bilateral hip replacements in the last 2 years. Since then, I have a general sense of imbalance; not vertigo, not dizziness. It is like my legs do not belong to me anymore. I have a friend who has experienced the same symptoms after her bilateral replacements. It did not happen after the first replacement, just the second, the same with her. I exercise in the pool 5 days a week doing water walking. Any ideas? Seems like a proprioception issue to me. I would welcome your input. Love your channel. Thanks. Nancy Watkins
I agree with most peoples comments, very good exercises. But do warn people that they may not be able to raise their legs even an inch if you’re glutes have been so destroyed in the operation. IT takes a lot of patience and will power until you lift your knees and legs as you suggest, especially right after the operation.
PLEASE WEAR NON-BLACK CLOTHING FOR BETTER VIDEO CLARITY. Your channel is one of the very best out there but is often limited by not being able clearly to see where exactly you are pointing or flexing/contracting due the black clothing. The white shirt and pillow today was great, thanks!

Going to PT now for spongy glutes that has caused me debilitating chronic lumbar pain. I find a shorterheight clamshell more effective.
