and before his last moment (i think?) he saw fredbear and so he died (i think?)

yup. he burned down every remaining freddy's souls and let the fire kill him too, since his promise with fredbear was fulfilled. that's why fredbear appears in the ending, to say jack his goodbyes for everything he's done. the graves in the ending are of the 5 main characters (jack, dave, peter, dee and steven), showing that they are resting in peace now
