      Try doing it with a small rubber ball, guys. It works better and spares your knuckles.

      Yes but what if you have elongated Piriformis syndrome?? These stretches will make it worse. You have to address what type of Piriformis syndrome you have before you just start stretching that muscle.

    If I could crawl through the screen and give you a big old kiss I would! I’ve been dealing with lower back pain for 8 years and after countless doctors, MRI’s, X-Rays, injections, physical therapy, chiropractor treatments, etc, I was still in so much pain every day. I finally found a new doctor who diagnosed me with Piriformis Syndrome and I’ve been trying all different finds of exercises which helped a bit but never gave me any relief.Well I just did this for the first time and I could feel that knot let go and once it did, the pain went from a 9 down to a 3! I’m going to do this a few times a day to get that muscle to let go forever but thank you so much! You’ve just changed my life!!

      When you find the true remedy to a root cause you know it. I had one of these oh my gosh moments myself after two years of pain and then it was gone in 20 seconds I spent the next two hours just reading the comments of people who felt like I did

    Most meaningful video I've seen out of the 1000s of videos that show how to get rid of piriformis pain.. I'm definitely going to try this out.. will post an update again after I do!

    Thank you so much for this video! I'd been having chronic pain in my right leg for a long time. This week it was especially bad. In finally researching this through a chain of videos, I learned of this syndrome and did the self-testing, which revealed the problem. And now your video gave me relief of pain today! My piriformis was very sore. I'll keep working with it and doing the other exercises Iearned. I'm very grateful!

    I was skeptical, of course..I thought I was having issues with my sciatic nerve but this fit better. I did this technique and within 20 minutes I could actually walk again!!! Far less pain

    just found you video. Thanks for putting it out there. One question, for someone who has a full hip replacement, what would you suggest instead of crossing your leg over the other knee and pulling back to identify the piriformis muscle. Any suggestions on other exercises that also might help? Thanks again!

    I've been in agony for upwards of a month and have a Spartan Race in less than a week. I've done every stretch under the sun, but this knuckle technique is the only one that has melted the pain away. What an absolute godsend. THANK YOU.

      Been having sciatica for years, it didn't always hurt just couldn't straighten out my leg
      I've been kinda doing that thing with the knuckle and massaging my buttcheek every now and then i guess my body's been trying to clue me on this the whole time

    Dr Mandell, what about those cases where the sciatic nerve bisects the piriformis? When I did this technique some years ago it mode things much worse - which is partly why I believe I'm one of those more unusual cases. Are there any alternative techniques?

    Ive been misdiagnosed for bulging or herniated disc before. Sciatic nerve pain has been consistently bothering me. I recently started watching videos about a week ago about piriformis syndrome. did all the stretches and broke up the fascia in the muscle and instantly noticed the pain was gone and since then ive been a new man. Im keeping up on my stretching and starting to slowly get back into weight training.

    i have tried all kinds of therapy and supplements and still have pain in my hip , this has been going on for 3 yrs i finally had an mri scan and it showed nothing wrong with my discs and hip. your vid showed me exactly what my problem is , after trying the trigger point release i was literally pain free for a while. i now know what my problem has been , so a big THANK YOU to Dr Mandell for this excellent vid , that has given me confidence in knowing what the problem was and how to resolve it . btw i never took any pain killers and i am treating the inflamation with circumin and serrapatase !!

      MRI only shows stenosis, didn't mention anything about piriformis syndrome. Ha ha... I am so happy that I do my homework. My right side of butt muscle is sore, now is released. Thank you to Dr.Mendell. 🤗

    So I was not going to put that much weight on my fist because that would not be good for my hand. So I laid down on a mat on the floor and used one of my dog's hard rubber balls. AWESOME!!! Wow I was able to dig in good and my tight/sore piriformis does feel much better. I'm going to be doing this every day.

    Electric heating pad will cure it fast.. it two months of every day sitting on the electric heating pad... Stretching does work but try stretching and sitting on the heating pad... Promise y'all this will definitely work.....

      I'll try that starting today and then I'll post a message here to let you know if it worked. Been dealing with the pain & discomfort for a month now. I thought was probably caused by prolonged sitting and poor posture while working (I spend a lot of time in front of the computer)...AND perhaps by going for a steal and diving for the ball then suddenly getting up (after I picked up the ball) and then running towards the hoop during our ballgame.

      Brandon Tucker yes mine is on my left side and is incredibly weak. is not until I get moving at work that I gain any strength.

      2011-2012 - Yes. I couldn't put weight on my right leg. I couldn't drive or lay in bed without being completely miserable. I had 2 falls during this period due to this issue.

    my ct scan showed i have s1 prolapse . but i also think i have this problem as a lucky bonus 😭 . im also finding it incredibly hard to have a shower because when that warm water hits my body my piriformis muscle contracts so hard im screaming in the shower .

    I have been treating my work related (sitting) lower back pain for years with chiropractic, yoga, gym time and stretches but always focusing on the lumbar. After your concise explanation I tried this method and experienced so much relief after only a few minutes that for some time I could only weep. Thank you so much for sharing your knowledge, I am still in disbelief.

    Whaaaat? How is this even possible? Been struggling with sitting/butt pain for over a year now, went to chiro, physio, and several sessions of massage but did not even help at all. I don't know if I will laugh or cry after several minutes of trying this technique. Thank you so much Dr.

    Thank you this is the best video I have found... instead of using my knuckles I’m using a tennis ball and already feel relief. I’m going to do this daily even several times a day until it is gone. Most videos are dumb stretches that don’t do anything, some are useful but again this is the best video I have found it makes sense that you need to loosen the piriformis as much as possible.

    Thank you Dr. Mandell, I'm definitely going to try this. I woke up Tuesday morning with horrible pain in my lower back and hips. It went from pain to worse pain and stiffness. I can't even reach to clean up using toilet. I'm taking huge amounts of pain killers just to function. I ran out of Humira two months ago, taking for Ankylosing Spondilitis, while waiting to meet with new Rheumatologist. Then on Tuesday I noticed some severe redness on my abdomen, which was diagnosed as Cellulitis. Dr told me I couldn't restart the Humira until finish the 10 antibiotic. In the meantime I'M FALLING APART! I can't bend over to reach the floor, and getting up from a seated position is hell. Hope this exercise will help. I'm soooo busy this weekend with two important events to lead on Sunday. Thanks again.

    Whenever I stretch my piriformis it just hurts it more? And it hurts the most whenever I do a deadlift position could someone plz help I'm only 14 and I've had this for over a year..

      Do what the Dr says, but use a baseball or softball instead. Hold it for 30 seconds to a minute. It will hurt like hell while you're pressing it. But after you stop, the pain will melt away

      I have had sciatica and piriform syndrome from taking my workouts to the next level, but I found out my lumbar and glutes are weak comparatively speaking...and the yoga stretches have worsened it since I sit alot at work, which stretches and weakens the glutes already...
      Try working and strengthening your glutes and lumbar region

    I can attest to the effectiveness of this technique, as well as the stretching exercises that Dr. Mandell has demonstrated in previous videos. What relief! I had never had sciatica before and suffered for several weeks before finding these videos. After performing these exercises and techniques, the pain quickly dissipated and ultimately was totally gone in little more than a week. Can't thank you enough Doc!

      That's awesome. I've been doing this stretch for 3 months now, and my pain is less, but still becomes excruciating when I put weight on my leg and try to walk. Sometimes I can make it like 30 feet before it hurts, but then it hits me like a freight train. Every night I do this stretch, plus a few others, but it seems to only have minimal effects.

      Bryan, then you probably don't have pain due to piriformis syndrome. Perhaps it's a bulging or herniated disk. Have you tried exercises that are similar to a cobra pose in yoga (lie on stomach and arch back)? This is supposedly good for this. Again, there are other videos regarding this issue. My issue was piriformis syndrome, I'm sure.

      Well I can occasionally feel a muscle, really seems like its the piriformis because its in the exact same spot as in the anatomy picture, it twitches and spasms, it feels really weird.

      Dr. i got very bad arthritis on my knees i was told to get knee replacement which i am not willing to do. please if you have any remedies or exercise to ease my pain.

      It's likely the sciatic nerve that is causing you pain attributed to piriformis muscle.

      mxd6 Periformis muscle pain is nothing like sciatic pain though. Quite destinguishable. I have both. And different areas too. If it shoots down your leg its the nerve, if its going across the butt diagonlly almost horizontally that would be performis.

      Any activity that alternates weight on one leg, then the other (like walking or standing for long periods) will exacerbate pain in the hip area.

      Look for vids on Sciatica Exercises. Do them daily and feel the difference. The older we get, the more we all need to stretch and exercise to feel better.

    I’ve heard a different take on this- on how you are not supposed to compress the piriformis because it’ll stretch it further and that sciatic nerve underneath is even more compressed. Please clarify. Thanks!

    This video hit the nail on the head. I’ve been suffering for a week now with my hip the back of my leg and the pain going down my leg and what seems like down into my knee and my shin. I thought we’ll maybe it was my sciatica nerve, but there was something different about it. This was brought on by me sleeping on a couch for a week. I was housesitting and all the bedrooms are upstairs and I chose not to sleep up there because I’ve had back surgery about six months ago and I’m still healing and I hate the stairs anyways. But my last night of housesitting I ended up with this pain in my hip, I thought it was coming from my back but then the pain went down to my leg. So I’m glad I’m the type of person that likes to research things and I’ve come back to your TH-cam channel with many things and I’m happy to say that you are right on. I don’t have a lot of money to go to a chiropractor so I won’t go. I do have a physical therapist, but if I can correct this at home then I will do that. Besides,I go to a physical therapist to help me learn new stretching techniques that I follow up at home anyways with. So I am going to finish your video and give this a try. Thank you Dr. Mandel.

      Google Dr. Tim Tollestrup. He pioneered the surgery for piriformis muscle removal. The surgery gave me instant relief 😉

    Excellent video. ...I'm getting an MRI today because of severe pain in lower back. Had to have spinal fusion surgery after my surgeon found out that my vertebra were so badly "mashed " two years ago . Now 2 years later I'm having different severe pain ...can not put weight on right leg without excruciating pain !!!!! Cramping like a toothache...almost complete disabled! !!!!
    I heard of pitiformis pain before. .. now since watching your video I'm making sure my surgeon completely checks out the Piriformis muscle .ThankYou again for this great video. . When you explained about the piriformis ... I said THAT is me and the ice makes it feel so better. .I'm going for the MRI but I am going to proceed with caution! !!!

    The problem I had was that my piriformis would tighten up after 5 to 6 hours of sleep and clamp down on my sciatic nerve and getting outa bed could turn into a 45 minute affair. Once I walked it off normal motion was no problem but doing these types of exercises did nothing to prevent my piriformis from tightening up as I slept. The fix was to not sleep long enough to give my piriformis time enough to tighten up. So ... I'd get up after 4 to 5 hours and start my day. Took a couple of weeks to get rid of it but the tactic worked whereas the exercises didn't.

    I want to Personally thank you Dr. Mandell , your brilliant explanation and solution to these back pain helped me a lot . After 6 years suffering from lower back pain , tightness glutes and hamstrings, I spent 2-3 yrs with chiropractor nothing changed. Finally I took your advice into consideration and practiced few times and realized a big changes, and the pain start to disappear slowly. 👍🏽

    I am suffering from Piriformis syndrom right now. I have severe multi level spinal stenonsis, and one area is the L5 & S1 area. I'm not near as active as I used to be because of this stenosis and arthritis that has developed. But I have been managing it with a chiropractor, ice, and Ibruprofen when needed. But now with the Piriformis area inflammed, there is really little what my chiropractor can do. But it is crippling pain. I have been on ice packs, anti inflammatories, pain killers and muscle relaxers for just a couple days now. Not much of improvement, if any at all. It is exhausting because I cannot sleep. I can walk about ten steps and that muscle cramps up and about brings me to my knee's. I pray...these meds kick in and the inflammation goes away. Very, very painful.

    No, no, no... Please check out Physical Therapy and Johnson channel... He has given me my life back... In two days... After 3 months of pain... Do not stretch the muscle, its the opposite of what u need to do... This is NOT helpful....its a 2 second relief.. Ur only aggravating the muscle... Do not do this...you need the muscle to contract, not lengthen....I mean no disrespect to this doc, I just want to help as I know the misery of this condition and I cant believe Im out of pain. At least give it a go if ur suffering and see if it helps. I hope it will...

    Your video is incredibly helpful, when I put my fist under my buttocks it is incredibly painful? I am going to continue to do the stretches and follow your instructions. I certainly hope it helps because I'm going into week 4 of this and it's almost unbearable

    If muscle relaxer like robaxin helps alleviate the pain does it rule out or support Piriformis? I had MRI’s a week ago, but waiting on the results, but the muscle relaxers combined with Tylenol and Motrin are helping control the pain. It’s not a cure or getting rid of it as it does come back.

    Strengthen the gluteus minimus which is the opposing muscle to the piriformis & that will help massively & give the piriformis a rest when it is over working,look up the wobble walk with bands which is brilliant for working the minimus,it works for me.

    What if the patient not able to do that stretch due to excessive pain and spasms? The pain feels in hip and left of thighs and in calf. The patient had gone a disketomy endoscopy surgery. The dr. administrated epidural steroid injection after one week to provide relief from light pain But after that the condition deteriorated further and pain started appearing in hip area, thigh and calf. Even dr. not sure about what causing this much pain. He asking to go for an open surgery for the same disk. Seeing your video and more video related to Forminal stenosis, supine piriformis and spinal stenosis. I have found that symptoms seems identical to things discussed in the video.

    I've been doing this with a small massage gun for a week. Along with the stretch it gives some temporary relief. But why won't it heal, It's rinse and repeat. Been over 4 months now.
