    You two characters are my people. I work in emergency medicine (30 years now!) I never dreamed our healthcare system would be in the shape it’s in now. It’s truly embarrassing. I can’t tell you how thankful I am that you are both willing to take time out of your busy schedules to sit and share your thoughts and opinions with us. I joked last week about “sanity restored” but, there is a lot of truth to that. I don’t necessarily NEED the validation to know I’M not the crazy one here but, it is so appreciated! Much success to you both in all you do!!!

    Congratulations Dr P on your teaching award! Students are so fortunate to have you as a professor and we are fortunate as an audience. I found the UMN oath to be very depressing and to be forced to recite something written by a handful of people to be further depressing. Nothing is perfect but tradition and ritual in this case are important to preserve bec the oath became meaningless.

      My father-in-law had a perforated bowel due to a botched colonoscopy. It pretty much ended his independence because he couldn't keep control of his bowels anymore. They had to remove a section of his colon due to the perforation and it would have killed him had he not prepped so well beforehand. My current doctor ordered me a colonoscopy and I have refused. I'm very biased with no family history.

    Not surprised about Dr. Prasad’s teaching awards. Both of you are great at explaining to non-experts-such an important skill and the test of real mastery. You don’t use jargon and you make people who haven’t spent years reading about a topic feel like they understand it, based on fundamental principles of logic, biology and well-designed scientific studies.

    Glad you discussed the anesthesia issue, as it's become an issue in my treatment options. (Long story short): In my 40s in NJ with IBS and inconclusive ultrasounds, I prepped for the colonoscopy. It wiped out my electrolytes, lowered my already low BP and pulse (to 80/50, 40 beats) so I couldn't get anesthesia. They refused to proceed without it, but sent me home with a 24-hr heart monitor and a nuclear stress test (verdict: 'athlete's heart'). Ever since, I've had only the fecal test. The IBS lessened once I got treated for hypothyroid.
    These invasive over-treatments and tests wouldn't have been done had I been uninsured, and I never would have paid for them myself. I've refused anesthesia and invasive procedures ever since, with no regrets. I will mercifully stop before detailing my mammography and PAP smear debacles. Love your show!

    Excellent discussion. GI NP here, and so I don't scope, but talk and treat patients with IBS, IBD, etc in clinic. I think the speciality itself is very interesting, but until we step back from paying more for procedures, this will get a lot of push back. I am biased, my Mom was one of those right sided CRC that resulted in hemicolectomy and I guess because we see so many cancers in our center, it makes us advocate for the procedure.

    You guys worry too much over the framing of the video and not enough about the audio, which is the point of the podcast, whether the video or audio-only version. Too frequently, I have to watch or listen with my hand on the volume control. On this one, I can barely hear VP, and ZD, you’re way too loud. Except I’ve noticed that Vinay is always has a much wider dynamic range when he talks. He gets suddenly loud when he gets excited about something and when he’s laughing. You could deal with the problem by routing both mics through an analog compressor on the way in, or by applying digital compression after recording. Then you can apply overall compression to the whole thing before rendering. Anyway, I love the podcasts nevertheless. But I think you’ll improve the accessibility (and ratings) of all your podcasts if you make them in a way that we can better hear what you’re saying.

    Capturing consistent audio is the biggest problem with the side-by-side format. Because the two of you are sitting next to each other, Vinay, you often move too far from the mic when you're looking at ZD. It causes your volume to drop significantly below his.
    The video look seems less conversational than when you do your shows at ZD's but I really like it for your solo videos. I should also emphasize that your audio is excellent in your solo videos.
    BTW, I'm not complaining. I'd follow the VDZP show if you streamed it on a single iphone under an LA overpass.

He has had some other cancers (liver, totally treated. prostate, to which the doctor said my Dad will probably die of something else before the prostate cancer) and complications (blocked bowel a year after his surgery, but we were able to find the liver cancer at that point.).

He’s thrilled, because he was worried that he would have to retire from his high-profile orchestra timpani job early. Of course, he has no idea how long this reprieve may last, or if it’s related to Covid, but this is the first I’ve heard of someone getting a benefit shortly after an infection. 58:00 yes the individual sentiments are wrong.
    "indigeneous" healing practices are not what the lawyers will focus on when medicaid patients show up at the ER and you paint a tiger on the swelling in their cheek and send them off
    also, this school explicity does not act as if the patient is in charge of their own treatment. Every student at UM is a patient who had to participate in an unpaid drug trial by Dec 31 2021 to patricipate in that seance
    white coats don't mean dominance in anyone's head except the admin of med schools like this. see previous example.
    talk is cheap. this video shows handpicked leftovers after anyone who would have walked out of the Milgram experiment already left last year. They will repeat, and do, anything the man in the white coat tells them to.

UNLESS they actually allow really traditional healing practices into theodern meds. bravo if they do that! and after hearing the new oath next to "white" coats...guess that's next

    • @stevec8861
      @stevec8861 2 ปีที่แล้ว +1

      Had mine done with no sedation. There's ~30 seconds of discomfort initially when colon is pumped full of air. Polyp removal is painless. Dental work is far more uncomfortable and have never been sedated for that. Have no idea why they push sedation for colonoscopy; it just drives up the risks and costs. Good to learn as side benefit that no sedation forces Gastro Doc to be gentler, which further reduces risk.

    • @margha6741
      @margha6741 2 ปีที่แล้ว

      You can still take IV sedation but a much lower dose. I just need that initial sedation for insertion. Then I am awake. Since I go to a colon cancer screening place they are not allowed to give alot of sedation, just the minimum. I was asked if I wanted none.

    Proper control of your wee wee 😂😂😂😂😂 I just did a ColoGuard. I’m 60. Did one at 55. It was easy ! Negative both times. I’m in healthcare and have seen too many perfs! I will pass on the scope and continue with the ColoGuard. ♥️

    Come on, guys, the FIT is easier than going through a colonoscopy! No prep, no torturing yourself the night before, no begging a friend to spend half a day taking you to a facility, no missing work, no post procedure restrictions from the sedation.

    This isn’t a central point but saying that Kentanji Brown Jackson wasn’t selected because of race/gender is a difficult claim to make when Biden said that was what he was doing. It’s not a great analogy.

    I much prefer your setup to ZDogg's because with yours, we see both of you all of the time. However, with ZDogg's, you're on different cameras that are switched between, so we only see each of you 1/2 of the time. It's just not as good.

    TO BOTH DOCTORS: Please consider reviewing the data just published on COVID-19 in The Lancet on October 15, 2022. Of particular note is the sentence under "FINDINGS" which speaks to natural immunity: Individuals with a history of COVID-19 infection were at reduced risk (infected ≥9 months before booster dose vs no previous infection; aRR 0·41 [95% CI 0·29-0·58]).
    Somehow, the authors didn't see this as a key part of the story.

    Vinay you make me laugh so hard. Together, I often spit out my drink. I mean, the "juice" with the colon test? I nearly had to pull over to the side of the road I was rolling. Love your views on covid. I agree with everything you have said over the last two years. Like the older woman who wouldn't get vaxxed, I often was dismissed and targeted (and I didn't even vote for Trump! LOL). I'm glad my feelings are shared by others, including both of you. Love both your shows!

    Another gripe re: Dr Z, I had a GI issue that lasted 3 months… I know pain - it was not imagined, just because it is difficult for doctors to analyze and diagnose GI issues doesn’t mean they are imagined, it means - you probably need a better doctor. OH Doctor my Doctor - think of your statement when you have a GI issue - you wll know true pain and difference in your gut.

      I don't think he was dismissing GI psych issues. He was commenting more about the brain gut working together (symbiosis.)which is very true. I had cdiff infection and terrible anxiety. There is lots of info re brain gut issues. I would love to hear more talk about this.

    My only concern with the colonoscopy study is that the SAD diet and American lifestyle are completely different from other countries. We cannot extrapolate from this study that even though they are all humans the environmental impacts are different and are typically what drive colon polyp/cancer formation. It would be interesting to see the same study performed in the US.

      My father had colon cancer at age 56. After his sigmoid was removed, he had to go for colonoscopies every 6 months. He had to drive an hour for the test so was always able to do them with no sedation. I have had 4 colonscopies. Usually take a small amount of sedation but am awake for most of it. Glad to hear this is good and that Dr less likely to perforate with an awake patient.

    Just thinking that in America our food is not the same as in European countries. I think we should keep doing the full colon based on our fatty fat intake. European dessert isn't even close to the amount of sugar we have in ours. This housekeeper used to bring us her baked goods and 😖. I'm guessing Norway is the same. When I visited Thailand... no aspartame in the diet coke. I don't know anything though.

    So I have a question, I've had 2 colos (I'm 40), because I'm at high familial risk. The first wasn't so bad, but I feel like they over dosed the knockout meds this last time. I've had 2 polyps removed. I know your not my doc, but how do I advocate for less knockout meds?
