  • When using a tracheostomy tube, one of the biggest concerns is keeping the airway hydrated. Normally when a person breathes, the upper airway warms and moistens air as it is inhaled. However, with a tracheostomy tube in place, the upper airway is bypassed. This means the air the person breathes in is very dry.
    Dry air will dehydrate the airway and cause the mucus to become very thick and sticky. The thick, sticky mucus can clog the tracheostomy tube and make it very hard to breathe. To prevent this from happening, it is essential to keep the airway moist and the mucus thin. One machine which helps hydrate the airway is called a cool mist trach collar.     Humidification is important for any kind of ventilation. Great of you to point out that a trach has a bit extra to consider, given the path that the air takes.

    My dad is possibly coming home with this, will it require electricity? Or will we have portable options

      Hello. You will need to discuss this with the medical equipment company and respiratory therapist. The one I have requires electricity, but there may be portable options available. It may be possible for your dad to use a HME (heat and moisture exchanger) when he leaves home. HMEs do not require electricity.

    I never liked cool mist! I didn't tolerate it when I use to use a trach collar. I preferred warm mist. My DME didn't have warm humidification set up for a trach collar. Now I'm on the vent all the time, so trach collars have no use to me anymore.

    What can someone use if they have a trache with HME and not on a vent. What are some recommendations to keep trache from being dry.

      You can run saline through a nebulizer. You can use a collar mist humidifier. You can use a heated humidifier. You can try a different HME. There are many different HMEs. I am not sure the HME you are using, but there are some which are larger and retain more moisture. Also, drinking more fluids during the day will help prevent the trachea from drying out. Running a room humidifier may also help.

      Hello. That is a great question. The HME is used to prevent moisture from escaping the airway. The cool mist trach collar is used to add moisture to the airway. The devices are similar. They both help keep the airway hydrated, but they also different.
      Cool mist trach collars are usually used when a person first gets a tracheostomy tube. Over time, the person is often weaned off the cool mist trach collar. Some may continue to use it only at night or for short periods of time during the day.
      The cool mist trach collar is also used for people who have a lot of secretions or have very thick secretions. The cool mist trach collar helps loosen the secretions and prevents them from clogging the tracheostomy tube.
      Cool mist trach collars may also be used when a person with a trach is sick with a respiratory infection. The cool mist trach collar will help keep the mucus thin.
      The HME is used to prevent the airway from drying out. If the airway dries out, the mucus will become thick and may clogged the tracheostomy tube.
      In short, the cool mist trach collar adds moisture to the airway. The HME prevents moisture from escaping the airway.
      Which device to use will depend on what needs to be accomplish. If the secretions need to be thinned, a cool mist collar can be used. If you just want to prevent the airway from drying out, the HME is a good choice.
