Exactly, if you're in Japan and looking for signs that say "restroom," it might say exactly that in English, but the Japanese will probably say トイレ (toilet) or お手洗い (washroom).
As someone who taught English in Japan for a long time this made my whole body hurt. But also... I kind of appreciate it when Japanese creators depict Japanese people speaking/attempting English. It makes Japanese kids actually stop and consider if it might be worthwhile for them.

I love how this just randomly switches back and forth between a feeling of "wow, for non-native speakers (especially high-schoolers, etc), those Japanese characters are actually doing really well with their English", and then "for God's sake, if the character is supposed to be American, _find a voice actor who can actually speak English instead of just mouthing the sounds somebody else wrote down for them!"_ (I met a number of people when I went to Tokyo who were actually able to speak English fairly naturally. There have _got_ to be at least one or two VAs in _the entirety of Japan_ who can do better than this!)

I doubt they’d hire a Japanese actor just to say a line in a dub. Most dubbing companies are in western territories so idk why you bring up English actors in Japan. That makes no feasible sense in so many ways.
Actually I have one example watch black lagoon English dub when Rock returns to Japan as Balalaika's translator, even if you are an English subtitles anime watcher, you will spot the difference.
But we have scenes of english actors trying to speak other languages, and sometimes it's really cringe
