In Hokuto no Ken episode 87, titled "Showdown at the Hokuto Renkitouza! No One Can Stop Them Now!!," the final battle between Kenshiro and Raoh takes place [1]. The episode originally aired on February 19th, 1987.
Synopsis:
The fight between Kenshiro and Raoh begins, with Kenshiro using Musou Tensei to dodge Raoh's punch [1]. Raoh, having also learned the technique, avoids Kenshiro's finishing strike by using Musou Tensei himself. Raoh reveals that he has a heart filled with grief and points to a statue holding Yuria [1]. Raoh destroys part of the statue, but Kenshiro catches Yuria and leaves her with Bat and Lin, who recently arrived at the Hokuto Renkitouza [1].
Meanwhile, Rihaku and his troops observe Raoh's forces, who are throwing away their weapons and being reunited with their loved ones [1]. A bright light shines on them, and they see the big dipper in the sky, signaling the beginning of the final battle [1]. The battle between Kenshiro and Raoh continues, with both fighters using various techniques against each other [1].
They both use Musou Tensei simultaneously, causing the ground to rip up as well as themselves [1]. Raoh realizes that since they are both masters of Musou Tensei, other techniques are now useless, and it becomes a contest of brute strength [1]. Bat and Lin notice that the two fighters don't bother blocking attacks and show no signs of hatred [1]. However, Lin observes that they seem to be becoming transparent and sees the younger Kenshiro and Raoh fighting [1].
In a flashback, it is revealed that Kenshiro opened Yuria's heart while Raoh drew her out [1]. The episode ends with Raoh unleashing his touki, but Kenshiro steps forward and makes Raoh's touki retreat [1]. Kenshiro tells Raoh that the battle has come to an end, and the next blow will decide their fate [1].
Fist of the North Star, the only 80's anime so violent that they had to wuss out a bit and have guys explode in radioactive milk to not get the series banned for the metric ton of gore it has XD

@dragonslover It's a kiai, a collected burst of sound and air to give more strength to your movements and/or intimidate your opponent. The latter obviously isn't needed since they'll be exploding soon, and since Hokuto Shinken is a breathing art, based on the power from the lungs, a kiai's power boost would be exponential.

I prefer to believe they were born this way :D Also, you can find infinite supplies of Kenshiro's tops, because for all the 109 episodes, there are one or two at most where he doesn't tear up his clothes and only a single episode where he sits behind a rock and sews them up. I like '80s anime logic.
