    For the next lesson we start our discussion of sedation in ICU patients and ICU sedation medications. Sedation is something that is used frequently in critical care and it has many potential uses and benefits, but some associated risks.
    We start the lesson of talking about what sedation is and what it can do for our patients. From there we move on to talk about the many different reasons that we would use sedation in the ICU and break down each of those reasons and explain a little bit more about each one. Finally we finish this lesson up talking about our monitoring of patients on sedation, specifically talking about the Richmond Agitation Sedation Scale (RASS).
    Hopefully after watching this lesson you will have a good foundation of understanding about ICU sedation and the purpose of its use as well as the potential benefits that can come from it. In the next lesson we will begin talking about the many various forms of sedation medication.

      I have a question; I had an upper endoscopy and I was given 100 Micro grams of Fentanyl during the 20 minute procedure, and Doctor added Versed for anxiety. When I woke up from the sedation I had major dizziness, and Nausea, and headache as I wanted to throw up really bad when I was taken to my car as a passenger of course, but my question is was it the Fentanyl that caused the bad reaction? or was it the Versed? Just to next time so I don't ask for Versed again due to nervousness?

In the current pandemic, this is very useful for surge doctors and nurses who do not have the full grasp of what sedation is all about and how patients are being managed with them.     I’ve been in ICU in a relatively low acuity unit for a few years and I’m looking to step up my game, get my CCRN, and begin working in higher acuity facilities.       Thank you! I of course highly encourage you to work towards getting in to the ICU. I may be biased, but I absolutely love it. I do have a couple videos where I talk about some of the cardiac assist devices (I'll link them below), but at some point I will probably make some videos specifically to those devices. Not sure when that will be though.

    delirium is very scary. i woke up from getting an aortic graft and thought the doctors were trying to murder me while they were extubating me cause i felt like i couldn’t breath and didn’t know what was happening and had to be held down

    Hi, you said that..we need to make sure that the patient has a good relief from pain before giving sedation..but how can I know if the patient has an adequate pain relief and now need only sedation? Thank you

      Look for things liked increased HR, grimacing, stiffness with turning or positioning, eyebrow furrowing. Also, just thinking about what is going on with them and whether there might even be pain. Obviously post surgical or trauma we would expect that. Other situations may or may not.

    Hi! I've been wondering for a long time: at our hospital we don't give midazolam to patients with liver cirrhosis (out of fear for agitation, less metabolism), but we do give diazepam as a drug against delirium in these same patients. Is this because diazepam has a long half life? Thank you!

      So I'm not real sure the reason there. There are many different schools of thought on the use of benzos with delirium.
      As for not giving midazolam, the reason is potential buildup of medication due to improper metabolism. Lorazepam is a good alternative benzo as it does not have active metabolites and is less sensitive to general liver dysfunction.
      You should ask your docs their reasoning in those situations and report back what their though process is.
      For liver patients, what med do you see used for sedation/agitation?

      Typically we use RASS or some form of sedation scale to determine if more or less sedation is needed. Then you titrate amount and frequency based on the order.

      It can, although from my understand and what I've seen on it, short term. So you wouldn't run the risk of being dependent long term, aka outside the hospital, but certainly if run long enough, high enough, we have to slowly reduce down as they very much can experience withdrawal.
