it could be allergies, I sometimes cough like a motherfucker too for the air being slightly too unpure to my body's liking
the coughing gets worse with lack of sleep, too. So maybe that could contribute to his condition as well unless he doesn't stay up late at all

Well to be fair, the damage ramps up so high that you're more likely to explode than actually tank it if you're under 30%hp. That ability is an execute 8 times out of ten.
