For the same reason IEM has a micro opening somewhere depending on the design to cancel out the pressure while wearing even if you're in a flight

Nice idea, i usually put my wired earphones loose. I put them loose so that air goes in n out without problem. Though this has a very high risk of falling and you will also hear background noises. I only put them loose if i am sitting and using.
