I've just had my third total knee replacement in my left knee since June 2022. It's a long story, I won't bore you with all the details. But, here I sit, 7am, I haven't been to sleep all night. It's like "am I ever going to sleep again?" I hope so. I love the humor you two doctors have. You crack me up, thank you, I needed that! 😊🤗😅

Tell your patients it is a whole lot easier to sleep on your back with both legs elevated instead of just one. It took me a couple of nights to figure this out.Greatly improved my sleep.

Tried sleeping on my back with a wedge pillow body would not accept it. I now sleep on my side with a pillow between my legs and another on my abdomen/ chest. It has worked for the last 3 nights.

I started using a wedge for both legs 5 days post-surgery (THR). Great results for this 74 year old female. However, I have to get up 4-5 times in the night for the bathroom, waking up Hubby to help me up, then back into bed. Why is bladder so active in the night when I’m on my back? I’m 14 days post-op. No pain, so no pain meds. Using a Walker. Starting PT this week.

You guys are so cool. I had my first knee (a Smith and Nephew, non-robotic assist TKR) April 1, 2024 and my other knee July 10, 2024. You are right on about the 1-2 months. Today, August 19, I finally had a night of sleep that I didn't wake up hollering because my newest knee was in a bad position! I am so thankful for my knees. I lost 112 pounds, got two new knees, am 61 years old and just now coming off the sidelines to enjoy a more active life. And do I ever have plans!

As an old night nurse, patients couldn't sleep post op due to pain and, over stimulation from the Staff, lab work, PT, visitors etc. We would medicate them and teach them slow breathing exercises, simple meditation to unwind. When I rehabilitated my family members, around nine at night was wash up time, meds, a small snack and no technologies!! Not sure what you do in Toronto, but for four weeks here in Maryland for TKR, they had to wear the leg cradle and wrap the knee in circulating ice water!!! Great. All about positioning. Took them a good fourteen weeks to truly sleep through the night with no meds or ice. Love when you guys bring up topics that most Docs scoff at or give flippant responses to said questions. Well done!

One fellow asked if I could sleep through the night yet. (I'm 3 weeks out from the operation) I'm still not sleeping the night through, but I think it's getting better. I am watching your TKR videos right now during my “night shift” as my mom’s caretaker. It’s day 12 after her surgery and the nights are the worst. My sister-in-law and I were taking turns but then I decided to take the graveyard shift for the next week straight (it’s hard to flip flop). I just gave my mom her meds and melatonin. She is currently snoring away but I’m fully alert for her if she wakes up. I also watched your top 5 mistakes video and read all of the 3K comments to gain perspective on her recovery. I am really touched by the fact that someone running this channel takes the time to respond to comments, even for older videos. New subscriber here ❤

Life circumstances have had me in trauma mode/ anxiety & depression and I’ve been hopeful that fixing my knee will help by getting me back to the hiking trails I love so much, and which serve as my mental health “therapy”. First Ijust want to say that you guys are so entertaining ! Next month will be one year since my first knee replacement and ten months since number two. I did have some trouble sleeping the first few days. Largely because of pain. But it did not take long to sleep decent. I have been back to work full time on my feet for 6 months now and every thing is great. Yes my quads still get achy and the surgery scar area is still waking up so I get new pains now and then . But the over all improvement over not being able to walk at all is so so so over the top wonderful I just can not begin to express it. At 56 I am no longer walking with two canes and a wheelchair is no longer a certainty !!! I just want you guys to know how great it is to have you here for not only entertainment but very much needed encouragement !! Please continue to be a beacon of hope for those wrestling with fear and uncertainty after surgery. Let them know life will get better !!!

After one hip and two knee replacements (all very successful) in the last 16 months, I agree with the two months of sleep degradation. Fortunately, at around that point each time, my sleep improved significantly. --- Keep up the great videos. Aside from the entertainment value, after watching your videos in preparation for my surgeries each time, I went into surgery with much less anxiety and reluctance than I likely would have otherwise.

Thank you - for the honesty. I have some experience to share. Both knees and 2 hips replaced, the second one done again 9 weeks ago and the first (20 years old) to be replaced next year... Managing post-op sleep/position, pain and medication reactions are real for many. Pillow placements, low tech equipment for getting in and out of bed independently, separate beds/room for first few weeks can help- just concentrate on you, balancing the right amount of pain relief, knowing what meds you react to and contribute to Your anxiety, bedtime rituals that work for you including slow controlled breathing. I'd love to read a well-designed research paper on self-management 4- 6 months post-surgery, based on surveys of real life experiences, excluding anyone who ran a marathon 3 months post surgery.

The older you get sometimes the harder it is sometimes to get to and remain asleep. I use an OTC sleep aid and have used Ambien every once in a while. I found that it was difficult to sleep on my right side after my right hip replacement; unfortunately that's my preferred sleeping side! As the docs note the pain does begin to mitigate with time but for those first weeks after surgery one should talk to their surgeon about alternative sleep aids to allow patients who may have issues sleeping. My primary care doc asked me not to take Ambien after my replacement but my ortho folks said it was ok...BUT you've got to take the precautions when you invariably get up at night to use the restroom...although I didn't need my walker from the git go (anterior "jiffy hip" procedure) I kept it next to the bed for a long time just to be safe. Thanks for a great topic that I'm guessing not a lot of us older patients think about before and after surgery. Merry Christmas guys!

3.5 months post 2nd TKR, first one was about 10 months ago. Been walking with no assist around the house, just taking it easy and being watchful. I use a cane or hubby's elbow outside for balance in case. Saw Surgeon last week, he said he is very happy with the way I am walking. We discussed some stiffness occasionally in my knees after rest and he explained that my knees were so bad and I used a walker for 2 yrs prior to both surgeries that my muscles will take time to strengthen and it is normal. Also, I am 73 and have been tired since the surgeries but it is slowly getting better as I get around more. As far as long term weight-bearing on a walker. I will send a warning to your patients to watch the actual pushing down for months at a time. I have now developed shoulder impingement in both arms (very painful) and am being treated for that at this writing. Best of luck to all of us on our journey..

First, I love you guys. I had a right TKR on October 25th and will have my right hip replaced on January 24th, 13 weeks after TKR. I watched your TKR videos prior to that surgery and am watching your hip replacement to help me prepare for that surgery. I have been in pain management since 2001. I have had 12 spinal surgeries and am fighting advanced stage prostate cancer. Chronic pain can cause depression and it’s nothing to be ashamed of. It took me a couple years to face up to it and talk to my doctor. He prescribed a mild anti-depressant and we talked about the fact that pain can cause depression and my quality of life improved and has been well managed. Please keep up the great work as your videos have been very informative and have helped me very much. Happy Holidays to both of you and your families!

I feel I'm going to have to buck up and drive through It. Sleep when I can and move as much as I can. It's good that my Pt gives goals to shoot for such as 90%s and now 100% flex.

I find this video reassuring and very useful. I recognized myself in the description of the reasons people cannot sleep well after joint replacement. Anxiety and depression are not to be ignored, they are very real. It is much better to know that your feelings and symptoms are common after joint replacement, to know that you are not alone and that there is hope that things are going to improve with time.

You guys should be required viewing, following not only total knee replacement, but for the myriad of other topics you cover. I am a month past a knee replacement and all of the symptoms concerning sleep have happened to me and continue to. The only strategy that has worked for me is wireless earbuds and a favourite playlist to distract from both pain and mental static. I’m sleeping more than before but less than normal. Looking foreword to getting back to normal sometime in the not too distant future. I’m going to mention this video to my orthopaedic surgeon for possible inclusion in in the hospital postop handbook. I have had a tri-malleolar ankle fracture/dislocation, a total ankle replacement and a severe shoulder fracture/dislocation.
I learned after the ankle fracture the importance of developing a sleep routine. Denise the Night RN is right. Turning off the phone/tv. I found that a play list of music designed to help you relax and sleep helped. As did lavender oil.
After the shoulder fracture, I was not able to have surgery for a week. The ER doc recommended one of the pain creams that have lidocaine in it at night, and the Orthopedic surgeon agreed. That made a huge difference.
Also if you are concerned about pain meds, talk with your surgeon. All my surgeons gave me the “good stuff” for a while, then backed me down to something not as strong so I had it if I needed it.

    Great job Docs!
I am 4 weeks out from my second total knee replacement, which was my 6th knee surgery. Not sleeping adds to the discomfort of any knee surgery. I have found cutting the pain medication in half helps. For example, instead of two tramadol tablets every 4-6 hours, I take one. During the day, two give me energy to power through physical therapy and my exercises, but I can usually take one at night. Plus, I put on a boring podcast to fall asleep to.
Regarding knee surgeries, depression always hits me the 5th week out. Since I know that, I make sure my primary care physician prescribes a mild antidepressant like Wellbutrin 6-8 weeks before knee surgery.
The above suggestions help; it is still not a pleasant ordeal, but the outcome and pain relief from having a total knee is worth it for your quality of life.

So finally getting some much needed shut eye. Had TKR November 2nd, so 8 weeks post op. Day 2 post op, my restless legs kicked in. I had RLS prior, but, not to this extent. Usually RLS would sometimes switch from one leg to the other, however, it was only on the surgical leg. And not only at night, but during some daytime hours. I was absolutely miserable! No sleep just up and having to walk behind my walker. Hadn't even started PT so you can imagine the difficulty so early on having to move around. It was always something grabbing, tightening on my leg causing insane movements, up, down, every which way. Was taking Pramipexole, then Rapinerole to no avail. Basically 7 weeks of misery! I'm now taking Lyrica and the .125 of Pramipexole with some relief. The RLS put me behind on my recovery as the RLS hindered my exercises at home. But, continuing with PT. If you suffer from RLS pre surgery, please talk to your doctor. I mentioned it but didn't follow through to find out what recourse to take if I got the RLS after surgery. Knowing what it's like to have restless legs, and that I would get it after surgery, I probably would have passed on the surgery. The leg pain I had before surgery would have been better than the TKR attacked by the restless legs. Really enjoy watching your videos! MERRY CHRISTMAS DOCS & HAPPY NEW YEAR!

My TKR was almost 2 years ago. I am doing great and it feels like my real knee and I do not have any pain or swelling. I am able to bike, hike and do whatever I want again. For me, I would always wake up about 1 or 2 am for the first 4 or 5 weeks. It was a combination of pain, I am a side sleeper and I could not get comfortable and my pain management would be about exhausted at that point. My worst nights were after therapy as I would always push myself really hard on those days. I would get up, take my pain meds, put the ice machine on it and elevate. Within 45 minutes or so and I would be able to go back to bed.

I am 12 weeks post-op and things are so much better. I can walk again at a good pace, have regained my balance, climb stairs, and best of all sleep on my side and drive. Unless I overdo things I no longer need any pain relief but I do still have leg swelling, aching and stiffness and continue to use ice packs when needed.
It has been an exhausting and terrifying experience that I was not prepared for (compounded by post-surgery complications). The sleep deprivation and psychological torture that comes with a total knee replacement has been by far the worst part of this process. I am so relieved that I can now sleep about 3-4 hours before aching and stiffness wakes me up. I just work through my stretching/bending routine, get up, walk about, have a drink, listen to music and do some sudoku ... that usually sends me back to sleep. I have actually come to appreciate and like my new knee - it does make clicking sounds but it is so strong and stable, my leg is straight again and I feel taller. I am, however, absolutely dreading having to go through this again when my other knee gets done in a few months. To everyone out there who is struggling ... don't give up, keep going, things will get better :)

The discomfort, pain, meds, position, ice, etc. - leading to lack of a restful night’s sleep - have made this surgery and post surgical recovery very very challenging. I had both my hips replaced at the same time, and my sleep was just destroyed for a month. It wasn’t pain because I didn’t have much, I just took Tylenol occasionally. It wasn’t stress or anxiety - I wasn’t really stressed. I just couldn’t get comfortable. Fortunately other THR patients had warned me about this, so it didn’t cause me stress, but it really was tedious. I watched late night infomercials and ended up buying things I never would have otherwise! I do think when nurses do the discharge counselling they should mention it, and assure patients it won’t last forever.

I am 10 days out after hip replacement, its 2am and here I am. I just can't get to sleep. It has gotten to the point I begin to get anxious once the sun goes down as I know my anxiety is messing up my head and I worry about what happens if I can't sleep again? ugh! just wearing me out. I have had one Nite with 4-5 hrs. of sleep but usually I crash at 5am, till 8am and I feel awful. Thank you for this info, it gives me hope that I will eventually return to normal. Tomorrow, I end my meds for bacterial infection prevention, gave up on the oxycodone on day 2, so the just Celebrex and Tylenol, maybe then things will change. Love you guys!

THAAATTT'S RIGHT!!!! IT'S 1:24 a.m. and I'm watching this video, cuz I'm not quite a month out from surgery, AND I CAN'T SLEEP ... AT NIGHT!!! Yes, increased stress, anxiety, pain, depression, and frustration that I'm sleeping 2 -4 hours during the day, to compensate. Not waking till 10 a.m. It's crazy-making!!! But thanks for this video, because it's knowing this is a common side effect is very helpful.I will just have to wait it out. Y might also consider that those who were more active (as best we could), pre-surgery, are also experiencing restlessness too! Thanks for the video!!!

@@TalkingWithDocs ... Thanks for the encouragement. Helpful to know this is a common problem. The fatigue is so bad tho, that it's interfering with exercise and healthy eating. Have resorted to buying take-out salads and chopped fruit just to make it easier. I need to eat more meat, but what I enjoyed pre-surgery is all a turn off now. Oooyyy!!!!

I’m at week 5 and starting to sleep for longer stretches3-5 hours. No pain meds anymore, sometimes ice, but blessedly sleeping on my side! I just celebrate that I am sleeping better, feel rested and just relaxing with broken sleep and accepting it as a norm. My full sleep will come, no panic! Thanks for these informative videos. They make me smile and grateful for your work and the science that makes these operations possible!😃

I had TKR, left first and then the right about five years later. For a long time after the pain was no longer an issue I was plagued by insomnia for at least five months in both instances. My GP prescribed Trazadone. It worked but I used it only occasionally. I did not have depression or anxiety. I dreaded going to bed because I would face hours of lying awake before sleep finally came. The insomnia eventually went away. Both replacements are working well and my quality of life has improved significantly.

I am 10 days post thr posterior surgery and in my early 40s. The pain only lasted the first few days. The worst part is not being able to get comfortable or sleep throughout the night. Thank you for the video.

Melatonin puts you to sleep, may cause diarrhea next day but you'll feel more rested. Otherwise eat small amount of foods that cause sleepiness. I can't remember with either hip replacement if I slept or worried about it because I knew it would pass. Thanks for this video.

Bad to no sleep for me post both my hip replacement surgeries - which I had five months apart in 2022. My Restless Leg Syndrome went through the roof and so did the depression! My primary doc did nothing!! To work out the problem, I cut out any alcohol, no caffeine after 8am, increased exercise, and meditated. It's week 5post surgery, and my sleep is "slowly" getting better. Good luck everyone! You're not alone! ☮️

Thanks Docs! I’m day 4 post op THA and it’s frustrating to only get 2 or 3 hrs sleep at a stretch. The new muscle ache is different from the old Hip OA pain. Good to know it’s temporary. Love your vids-keep up the good work!
